目的:总结糖尿病食管癌患者的围手术期处理经验。方法:对50例糖尿病食管癌患者术前、术中、术后应用胰岛素控制血糖,使血糖稳定在理想水平。结果:50例患者平稳度过全麻开胸手术,术后呼吸道感染13例,切口愈合不良8例,心律失常7例,心功能衰竭1例,肾功能不全1例,低钾血症4例,经治疗后均痊愈出院。结论:对合并糖尿病的患者做好术前准备,围手术期将血糖控制在理想水平,加强抗感染,营养支持治疗,可减少术后并发症。
Objective: To summarize the experience of perioperative management of diabetic esophageal cancer patients. Methods: 50 cases of diabetic esophageal cancer patients before, during and after the application of insulin to control blood sugar, blood sugar stabilized at the desired level. Results: Fifty patients underwent general anesthesia and thoracotomy smoothly. There were 13 cases of postoperative respiratory infection, 8 cases of incision healing, 7 cases of arrhythmia, 1 case of heart failure, 1 case of renal insufficiency, 4 cases of hypokalemia , After treatment were cured and discharged. Conclusion: Preoperative preparation of patients with diabetes mellitus, perioperative glycemic control at an ideal level, strengthen anti-infective, nutritional support and treatment, can reduce postoperative complications.
