目的 探讨子宫内膜组织中的DNA含量和端粒酶活性在诊断子宫内膜腺癌中的价值。方法 以流式细胞仪及原位杂交方法分别测定50例不同增生程度的子宫内膜组织中的DNA含量及端粒酶活性。结果 DNA含量比较:增生过长子宫内膜与正常子宫内膜相比异倍体率及DI值(细胞半衰期)无显著性差异(P>0.05)。SPF值(S期细胞比率)有显著性差异(P<0.05)。子宫内膜腺癌与正常子宫内膜、增生过长子宫内膜相比异倍体率、DI值和SPF值均有显著性差异(P<0.05)。且三种指标均随着子宫内膜分化程度的降低而增高,各级间相比均有显著性差异(P<0.05)。端粒酶活性比较,增生过长子宫内膜较正常子宫内膜阳性率高,但无显著性差异(P>0.05);子宫内膜腺癌与正常子宫内膜、增生过长子宫内膜相比均有显著性差异(P<0.05);且各级间随子宫内膜分化程度的降低阳性率有增加的趋势,但各级间相比无显著性差异(P>0.05)。结论 DNA非整倍体是子宫内膜腺癌分子生物学的特异性标志。异倍体率、DI值及SPF值可能均在子宫内膜腺癌的诊断中有重要意义。可能是判定子宫内膜腺癌分化程度的指标。端粒酶活性增高在子宫内膜腺癌的诊断中有重要意义,可能成为早期诊断的标志。
Objective To investigate the value of DNA content and telomerase activity in endometrial tissue in the diagnosis of endometrial adenocarcinoma. Methods DNA content and telomerase activity in 50 cases of endometrial tissue with different proliferative degree were determined by flow cytometry and in situ hybridization. Results DNA content comparison: There was no significant difference in aneuploidy rate and DI value (cell half-life) between hyperplastic endometrium and normal endometrium (P> 0.05). SPF value (S phase cell ratio) was significantly different (P <0.05). There were significant differences in aneuploidy rate, DI value and SPF value between endometrial adenocarcinoma and normal endometrium and hyperplastic endometrium (P <0.05). The three indexes all increased with the decrease of the degree of endometrium differentiation, with significant difference (P <0.05). The positive rate of telomerase in endometrial hyperplasia was higher than that in normal endometrium, but there was no significant difference (P> 0.05). The expression of telomerase activity in endometrial adenocarcinoma was significantly higher than that in normal endometrium, hyperplastic endometrium (P <0.05). The decreasing rate of endometrial differention among all the stages had an increasing tendency, but there was no significant difference among all stages (P> 0.05). Conclusion DNA aneuploidy is a molecular marker of endometrial adenocarcinoma. The aneuploidy rate, DI value and SPF value may all have important significance in the diagnosis of endometrial adenocarcinoma. May be to determine the degree of endometrial adenocarcinoma differentiation index. Increased telomerase activity in the diagnosis of endometrial adenocarcinoma is of great significance, may become a sign of early diagnosis.
