本研究通过检测噬血细胞性淋巴组织细胞增多症(HLH)患者外周血中可溶性IL-2受体(sCD25)及NK细胞活性,探讨它们在HLH中的意义。应用酶联免疫吸附法检测20例HLH患者、15例正常对照者、20例急性髓系白血病(AML)患者及20例系统性红斑狼疮(SLE)患者外周血血清sCD25水平;用流式细胞术CD107a标记法及传统的LDH释放法检测HLH组及正常对照组外周血NK细胞活性。结果显示:HLH组血清sCD25水平较正常对照组、AML组及SLE组均明显升高(P<0.001);HLH组NK细胞活性明显低于正常对照组(P<0.05);将流式细胞术CD107a标记法与传统的LDH释放法检测NK细胞活性进行相关分析,该两种检测方法之间具有显著相关性(r=0.73,P<0.05)。结论:血清sCD25及外周血NK细胞活性检测是HLH重要的辅助诊断指标,流式细胞术CD107a标记法检测NK细胞活性简单、稳定、重复性高,有助于HLH的临床诊断。
In this study, we examined the significance of sIL-2 (sCD25) and NK cell activity in peripheral blood of patients with hemophagocytic lymphohistiocytosis (HLH) and their significance in HLH. Serum levels of sCD25 in 20 patients with HLH, 15 normal controls, 20 patients with acute myeloid leukemia (AML) and 20 patients with systemic lupus erythematosus (SLE) were detected by enzyme-linked immunosorbent assay (ELISA) The activity of NK cells in peripheral blood of HLH group and normal control group was detected by CD107a labeling and traditional LDH release assay. The results showed that the level of serum sCD25 in HLH group was significantly higher than that in normal control group, AML group and SLE group (P <0.001); NK cell activity in HLH group was significantly lower than that in normal control group (P <0.05); Flow cytometry Correlation analysis between CD107a labeling and traditional detection of NK cell activity by LDH release showed significant correlation (r = 0.73, P <0.05). CONCLUSION: The detection of serum sCD25 and peripheral blood NK cell activity is an important auxiliary diagnostic indicator of HLH. The detection of CD107a by flow cytometry is simple, stable and highly reproducible, which is helpful for the clinical diagnosis of HLH.
