目的探讨五分类全自动血液分析仪检测指标异常时,血细胞形态学观察。方法收集五分类血液分析仪检测中,白细胞(WBC)、中性粒细胞百分率(N%)、单核细胞百分率(M%)、淋巴细胞百分率(L%)、嗜碱性粒细胞百分率(B%)、嗜酸性粒细胞百分率(E%)、红细胞(RBC)、血红蛋白(Hb)、血小板(PCT)、红细胞平均血红蛋白(MCH)、红细胞平均体积(MCV)、红细胞平均血红蛋白浓度(MCHC)、红细胞分布宽度(RDW)、指标异常的标本576例,和182例上述指标正常的血标本,作为对照组,同时进行血涂片形态学观察并对比分析。结果试验组细胞异常检出率特别高,WBC总指数增高,并伴有N%、L%、M%、B%、E%、增高时,血细胞形态可观察到中性粒细胞核左移、空泡、中毒颗粒、异淋、幼稚细胞、有核红细胞,RBC、Hb、MCH、MCV、MCHC、RDW指标异常,可见红细胞大小不一,红细胞中心浅染区扩大。对照组异常检出率低。结论当血液分析仪报警时,临床上有白细胞增高,特别是临床上有贫血、肝脾淋巴结肿大、发热、等疑似白血病时,必须做血涂片检查。
Objective To investigate the morphological changes of blood cells when the five categories of automatic hematology analyzer were used to detect abnormalities. Methods Fifty blood samples were collected and analyzed by WBC, neutrophil percentage (N%), monocyte percentage (M%), lymphocyte percentage (L%) and basophil percentage %, Eosinophils, RBC, Hb, PCT, MCH, MCV, MCHC, RDW, 576 abnormal samples, and 182 normal samples were used as the control group. Blood smear morphology was also observed and compared. Results The abnormal detection rate of cells in the experimental group was particularly high, the WBC index was increased, accompanied by N%, L%, M%, B%, E%. When increasing, the neutrophil nuclei shifted to the left There were abnormalities of erythrocyte size, erythrocyte size, light-stained area of erythrocyte, and light-stained area of erythrocytes. Control group abnormality detection rate is low. Conclusion When the hematology analyzer alarm, clinically leukopenia, especially clinically anemic, hepatosplenomegaly, fever, and other suspected leukemia, blood smears must be checked.
