在海湾战争中,美军发生许多由地雷、大炮、炸弹、火箭炮和迫击炮弹片致伤的伤员,伤员多、伤情复杂。同以往战争相比,此次战争由轻武器致伤的伤员和创伤却较少。鉴于美军陆军医院阶梯后送的特点,许多医院要接受和治疗在其他医院已行清创的伤员。遗憾的是,这些伤员的创口往往在清创后均被包扎起来了。由于后送、转运的耽搁,清创后的创口不但发生肿胀,而且发生不必要的闭合。因此,创口5天后传统的延期一期闭合常难完成,以致于不得不依赖于矫形外科的帮助,但在野战条件下这是很难做到的。野战条件下美军在处理大批战伤伤员的过程中,采用持续性张力牵引封闭战伤创口收到很好的效果。他们利用这种简单的技术,
