选取158例2型糖尿病患者,随机分为A组应用门冬胰岛素30,三餐前皮下注射,B组应用普通胰岛素R三餐前30分钟皮下注射,联合普通中效胰岛素晚睡前皮下注射,C组应用门冬胰岛素三餐前皮下注射,联合甘精胰岛素晚睡前皮下注射,D组采用胰岛素泵持续皮下注射门冬胰岛素。结果:①A组空腹和晚餐后2小时指尖血糖均显著高于D组(P<0.05),而B、C组和D组三组间空腹血糖和晚餐后2小时指尖血糖两两比较无明显差异(P>0.05)。②C组和D组患者强化治疗前、后各时间点指尖空腹指尖血糖下降幅度大于A组(P>0.05),其他时间点4组间血糖下降差值两两比较无明显差异(P>0.05);③D组患者血糖达标时间最短,胰岛素用量最少,B组患者血糖达标时间最长,B组胰岛素用量最多;④D组低血糖发生率最高,C组低血糖发生率最低。结论:四种胰岛素强化治疗方案对2型糖尿病短期疗效均较好;胰岛素泵治疗血糖达标时间最短,胰岛素用量最少;门冬胰岛素30可做为胰岛素强化治疗方案之一。
158 patients with type 2 diabetes mellitus were randomly divided into group A with insulin aspart 30, subcutaneous injection before meals, group B with normal insulin R subcutaneous injection for 30 minutes before meals, combined with normal mid-dose insulin subcutaneously before going to sleep at night, Group C, insulin aspart was administered subcutaneously three times before meals, and combined with glargine subcutaneously before going to sleep at night. Group D was given subcutaneous injections of insulin aspart by subcutaneous injection of insulin pump. Results: ① The fingertip blood glucose in group A was significantly higher than that in group D at two hours after fasting and at two hours after dinner (P <0.05), while the fasting blood glucose in group B, C and D was no significant difference compared with those at two hours after dinner Significant difference (P> 0.05). ② In group C and group D, the decrease of blood glucose of fingertips was greater than that of group A before and after intensive treatment (P> 0.05). There was no significant difference in the difference of blood glucose among four groups at other time points (P> 0.05) .③The patients in group D had the shortest time of reaching the standard of glycemic control, the least amount of insulin, the longest duration of blood glucose in group B, and the highest amount of insulin in group B. ④The incidence of hypoglycemia in group D was the highest, while the incidence of hypoglycemia in group C was the lowest. CONCLUSION: Four kinds of intensive insulin therapy have a good short-term curative effect on type 2 diabetes. Insulin pump has the shortest blood glucose compliance time and the least amount of insulin. Insulin aspart 30 can be used as one of intensive insulin therapy.
