目的了解合肥市某医院门急诊处方质量、抗菌药物的使用情况及存在问题,为促进合理用药提供科学依据。方法于2010年6月至2011年3月每月随机抽取门急诊处方各100张,共计1 000张,依据《医院处方点评管理规范(试行)》规定进行处方点评和统计分析。结果该医院门急诊合理处方仅占38.9%,其中电子处方合格率(63.72%)高于手写处方合格率(31.65%),其差异具有统计学意义(χ2=75.66,P<0.001);不合理处方主要表现为不规范处方,占不合理处方的94.11%;抗菌药物使用率为53.80%,前3位的依次为头孢菌素、喹诺酮和大环内酯类,占抗菌药物处方总数的比例分别为40.07%、18.70%和8.18%。结论该院门急诊处方合格率较低,需重视处方质量,加强对处方及抗菌药物应用的管理。
Objective To understand the prescription quality of emergency and outpatient department in a hospital in Hefei, the use of antibacterials and their existing problems, so as to provide a scientific basis for promoting rational drug use. Methods A total of 100 outpatient and emergency prescriptions were randomly selected every June from June 2010 to March 2011. The total number of prescriptions was 1 000, and prescription reviews and statistical analyzes were conducted according to the regulations of “Management Regulations of Hospital Prescription Reviews (for Trial Implementation)”. Results The hospital outpatient and emergency department accounted for only 38.9% of the rational prescriptions, of which electronic prescription pass rate (63.72%) was higher than handwritten prescription pass rate (31.65%), the difference was statistically significant (χ2 = 75.66, P <0.001) Prescriptions were mainly non-standard prescriptions, accounting for 94.11% of unreasonable prescriptions; the use of antibacterials was 53.80%. The top three followed by cephalosporins, quinolones and macrolides, accounting for the proportion of the total number of prescriptions for antimicrobial drugs 40.07%, 18.70% and 8.18% respectively. Conclusion The outpatient and emergency department of the hospital prescription pass rate is low, the quality of prescriptions need to be emphasized, to strengthen the administration of prescriptions and antimicrobial agents.
