目的探讨二维超声和二维联合实时三维超声在孕早中期筛查胎儿畸形的价值,为临床推广无创伤筛查方法提供基础资料依据。方法回顾性分析中南大学湘雅医学院附属海口医院于2013年1月—2015年6月共4 929例产妇经产前二维超声和二维联合实时三维超声对比检查结果,并对比分析追踪引产或分娩的结果。结果引产或分娩后证实胎儿畸形151例,二维超声检出的胎儿畸形108例,准确率为71.5%,二维超声联合实时三维超声检出胎儿畸形135例,准确率为89.4%,差异有统计学意义(P<0.05)。二维超声联合实时三维超声在胎儿神经系统畸形、先天性心脏畸形、消化道畸形、唇腭裂、两种及以上多种畸形等的检出准确率要明显高于单独使用二维超声,差异有统计学意义(P<0.05)。结论二维超声联合实时三维超声检查可以明显提高胎儿畸形的筛查率,有助于优生优育。
Objective To investigate the value of two-dimensional ultrasound and two-dimensional combined real-time three-dimensional ultrasound in the screening of fetal malformations during early pregnancy and early pregnancy, and provide the basis for clinical application of non-invasive screening methods. Methods A retrospective analysis of 4929 maternal prenatal two-dimensional ultrasound and two-dimensional combined real-time three-dimensional ultrasound contrast examination of Haikou Hospital, Xiangya School of Medicine, Central South University from January 2013 to June 2015, and comparative analysis of tracing abortion Or the result of childbirth. Results 151 cases of fetal malformations were confirmed after induction of labor or childbirth, 108 cases of fetal malformations were detected by two-dimensional ultrasonography and the accuracy rate was 71.5%. 135 cases of fetal malformations were detected by two-dimensional ultrasound and real-time three-dimensional ultrasound with the accuracy of 89.4% Statistical significance (P <0.05). Two-dimensional ultrasound combined with real-time three-dimensional ultrasound in fetal neurological deformity, congenital heart malformations, digestive tract malformations, cleft lip and palate, two or more multiple deformities and other detection accuracy was significantly higher than the separate use of two-dimensional ultrasound, the difference was Statistical significance (P <0.05). Conclusion Two-dimensional ultrasound combined with real-time three-dimensional ultrasound can significantly improve the screening rate of fetal malformations, help prenatal and postnatal care.
