目的评估Xpert MTB/RIF技术在结核病诊断及耐药检测中的应用价值。方法对369例门诊初诊者的痰液进行荧光染色镜检、固体培养、传统比例法药敏实验和Xpert MTB/RIF检测,分析Xpert MTB/RIF技术对痰标本中结核分枝杆菌(MTB)及利福平(RIF)耐药性检测的灵敏度、特异性及一致性。结果以固体培养为金标准,Xpert MTB/RIF检测MTB的灵敏度为99.03%,特异度为83.76%,符合率为88.43%。以传统比例法药敏为金标准,Xpert MTB/RIF检测RIF耐药情况的灵敏度为100.00%,特异度为98.97%,一致性为99.01%。结论 Xpert MTB/RIF技术操作简便快速、结果判读简便,检测MTB和RIF耐药性均具有较高的灵敏度和特异度,适合在基层结核病诊疗机构推广。
Objective To evaluate the value of Xpert MTB / RIF in diagnosis of tuberculosis and drug resistance. Methods The sputum samples from 369 outpatients were detected by fluorescence microscopy, solid culture, drug susceptibility test by traditional proportional method and Xpert MTB / RIF assay. The results of Xpert MTB / RIF assay on the sputum samples of Mycobacterium tuberculosis (MTB) and Sensitivity, specificity and consistency of rifampicin (RIF) resistance assays. Results The solid culture was the gold standard. The sensitivity and specificity of Xpert MTB / RIF assay for detecting MTB were 99.03% and 83.76%, respectively. The coincidence rate was 88.43%. The sensitivity of Xpert MTB / RIF to detect RIF resistance was 100.00%, specificity was 98.97%, and consistency was 99.01%. Conclusion The Xpert MTB / RIF technique is simple, rapid and easy to interpret. The sensitivity and specificity of MTB and RIF assays for detection of MTB and RIF are suitable for the promotion of primary TB clinics.
