目的探讨复发性卵巢癌再次手术后并发症的相关因素和防治方法。方法回顾性分析1995年3月至2004年6月四川大学华西第二医院111例复发性卵巢癌行再次手术治疗患者的临床资料。结果复发性卵巢癌再次手术后并发症总发生率为25.2%,较初次手术术后并发症发生率(7.2%)明显增加,主要为腹部伤口感染、肠梗阻、肠瘘和直肠壁损伤等。<60岁组和≥60岁组腹部伤口感染率分别为5.1%(4/78)和24.2%(8/33)。术后全身支持治疗可使感染发生率由19.1%降至4.7%。结论复发性卵巢癌再次手术后并发症的发生与患者年龄、术后营养状况、手术方式以及术者的经验技能等有关,充分的术前准备和围手术期支持治疗可降低并发症的发生率。
Objective To study the related factors and prevention and cure of recurrent ovarian cancer after reoperation. Methods The clinical data of 111 patients with recurrent ovarian cancer undergoing reoperation in 111 West China Second Hospital of Sichuan University from March 1995 to June 2004 were retrospectively analyzed. Results The recurrence rate of recurrent ovarian cancer after surgery was 25.2%, which was significantly higher than that of the first surgery (7.2%). The main complications were abdominal wound infection, intestinal obstruction, intestinal fistula and rectal wall injury. Abdominal wound infection rates were lower in the 60-year-old group and in the 60-year-old group as 5.1% (4/78) and 24.2% (8/33), respectively. Postoperative systemic support therapy reduced the incidence of infections from 19.1% to 4.7%. Conclusions The incidence of postoperative complications in patients with recurrent ovarian cancer is related to patient’s age, postoperative nutritional status, operation mode and experience of the surgeon. Adequate preoperative preparation and perioperative supportive care can reduce the incidence of complications .
