目的报告1例银屑病并发臀部巨大黏液腺癌治疗方法及效果。方法 2014年2月收治1例64岁银屑病并发臀部巨大黏液腺癌男性患者。入院检查见左侧臀部肿物,大小约15 cm×10 cm,呈菜花样生长,伴大量胶冻样渗出。盆腔MRI提示左臀部皮肤区见不规则团状异常信号影,病变累及盆底双侧肛提肌及直肠后壁。分期行乙状结肠双腔造瘘术、肿瘤切除及会阴直肠肛管切除术、近端直肠闭合术以及游离植皮修复术。结果术后患者获随访3个月,其状态良好,植皮成活,创面愈合良好。结论对于银屑病并发肛周黏液腺癌,可选择腹会阴联合扩大切除术,并采用全厚皮片植皮修复肿瘤切除后的创面,可获得较好疗效。
Objective To report a case of psoriasis complicated with huge hog mucinous adenocarcinoma and its effect. Methods A total of 64 male patients with psoriatic giant cell carcinoma of the gonads at the age of 64 were admitted in February 2014. Admission examination, see the left hip mass, the size of about 15 cm × 10 cm, was cauliflower-like growth, with a large number of jelly-like exudate. Pelvic MRI prompts the left hip skin area irregular celiac abnormalities seen signs of lesions involving the pelvic floor levator ani muscle and rectum. Staging of sigmoid double-chamber ostomy, tumor resection and perineal and anal resection, proximal rectal closure and free skin graft repair. Results Postoperative patients were followed up for 3 months, their status was good, skin graft survival, wound healing. Conclusion For psoriasis complicated with perianal mucinous adenocarcinoma, the choice of abdominal perineum combined with extended resection, and the use of full-thickness skin grafts to repair the tumor after resection, can obtain better curative effect.
