目的:观察补脾胃泻阴火升阳汤联合美沙拉嗪治疗溃疡性结肠炎的临床疗效及对炎症细胞因子的影响。方法:选择符合纳入标准的溃疡性结肠炎患者195例,采用简单数字的随机方法分为对照1组,对照2组和治疗组,每组65例。对照1组给予美沙拉嗪肠溶片口服,1 g/次,3次/d(tid),对照2组患者在口服美沙拉嗪肠溶片的基础上给予陈李济补脾益肠丸口服,6 g/次,tid。治疗组患者在口服美沙拉嗪肠溶片的基础上给予补脾胃泻阴火升阳汤口服,每次100 mL,tid。观察3组患者治疗前后症状、大肠黏膜积分变化和细胞因子高敏C反应蛋白(high sensitivity C-reaction protein,hs-CRP),白细胞介素-6(interleukin-6,IL-6),肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平变化,统计临床疗效。结果:治疗组治疗后在证候积分(腹痛、腹泻、脓血便),大肠黏膜积分、细胞因子(hs-CRP,IL-6,TNF-α)水平较对照1组,对照2组均明显降低(P<0.05);治疗组,对照1组,对照2组临床疗效总有效率分别为93.85%,78.46%,81.54%,治疗组临床疗效优于对照1组,对照2组(P<0.05)。结论:补脾胃泻阴火升阳汤联合美沙拉嗪能够有效地缓解溃疡性结肠炎患者临床症状及大肠黏膜病变,改善细胞因子水平,值得进一步推广及应用。
Objective: To observe the clinical curative effect and its effect on the inflammatory cytokines in the treatment of ulcerative colitis with BuPiWeiYin Huoshengyang Decoction combined with Mesalazine. Methods: A total of 195 patients with ulcerative colitis who met the inclusion criteria were selected and divided into control group 1, control group 2 and treatment group with 65 cases in each group by simple digital method. Control group 1 was given mesalazine enteric-coated tablets orally, 1 g / times, 3 times / d (tid), the control group 2 patients were given oral administration of mesalazine enteric-coated tablets on the basis of Chen Li Ji Bu Pi Yi Chang Wan oral administration of 6 g / times, tid. Treatment group of patients given oral mesalazine enteric-coated tablets on the basis of the spleen and stomach diarrhea Yin fire Sun soup oral, each 100 mL, tid. The changes of symptoms, the changes of mucosal integral and the expression of high sensitivity C-reaction protein (hs-CRP), interleukin-6 (IL-6), tumor necrosis factor -α (tumor necrosis factor-α, TNF-α) levels, statistical clinical efficacy. Results: After treatment, the scores of syndromes (abdominal pain, diarrhea, pus and bloody stools), colorectal mucosa score, levels of cytokines (hs-CRP, IL-6 and TNF-α) in treatment group were significantly lower than those in control group 1 and control group 2 (P <0.05). The total effective rates of treatment group, control group 1 and control group 2 were 93.85%, 78.46% and 81.54% respectively. The clinical curative effect in treatment group was better than that in control group 1 and control group 2 (P <0.05) . Conclusion: BuBuGuiYinHoshuoYang Decoction combined with Mesalazine can effectively alleviate the clinical symptoms and colorectal mucosal lesions in patients with ulcerative colitis, improve the level of cytokines, which is worthy of further promotion and application.
