[目的]了解黔南州水族宫颈疾病妇女的人乳头状瘤病毒(HPV)感染现状,为对HPV防治提供流行病学数据。[方法]采用最新的HPV检测技术——第2代杂交捕获试验,对1859例来自黔南州水族宫颈疾病患者宫颈脱落细胞标本进行HPV检测及基因分型,分析HPV在黔南州水族宫颈疾病妇女感染率以及高危型HPV(HR-HPV)感染率,研究不同HPV亚型在不同人群中的分布,并进行Logistic回归分析。[结果]宫颈疾病患者HPV感染率为68.26%,重叠感染率为30.28%,在30～34岁组及40～49岁组出现两次感染高峰;HR-HPV感染率42.44%;HR-HPV亚型感染率排名分别是HPV16、33、58、52、31;以30～34岁组感染率为最高;宫颈炎症以HPV16、58、53为主,宫颈不典型增生以HPV16、33、58为主,宫颈癌以HPV16为主。多因素Logistic回归分析结果表明,吸烟、性伴侣数≥2个、初次性生活年龄≤18和孕次≥2次是水族妇女宫颈疾病HPV感染的独立危险因素。[结论]HPV在黔南州水族宫颈疾病妇女生殖道有较高的感染率及重叠感染率,不同宫颈疾病优势型别不同,故HPV感染应该引起社会的重视,加强常规检测HPV,以早期发现感染,及时干预。
[Objective] To understand the status of human papillomavirus (HPV) infection in women of aquarium cervical cancer in Qiannan Prefecture and to provide epidemiological data on HPV prevention and treatment. [Method] The HPV test and genotyping of 1859 cases of cervical exfoliated cells from aquarium cervical lesions in Qiannan Prefecture were carried out using the latest generation HPV test, the second generation hybridization capture test. The results of HPV test in Guizhou Qiannan Aquarium cervical disease Women’s infection rate and high-risk HPV (HR-HPV) infection rate were studied. The distribution of different HPV subtypes in different populations was studied and Logistic regression analysis was conducted. [Results] The HPV infection rate was 68.26% and the overlap infection rate was 30.28% in patients with cervical diseases. The infection peak appeared in 30-34 years old group and 40-49 years old group. The HR-HPV infection rate was 42.44% Type infection rate were HPV16,33,58,52,31; 30 to 34-year-old group was the highest infection rate; cervical inflammation HPV16,58,53 mainly cervical atypical hyperplasia HPV16,33,58-based , Cervical cancer mainly HPV16. Multivariate logistic regression analysis showed that there were more than 2 smoking partners, more than 2 sexual partners, initial sexual life≤18 and pregnancy times ≥2, which were independent risk factors for HPV infection of cervical diseases in aquatic women. [Conclusion] The infection rate and overlapping infection rate of HPV in the genital cervix of Qiannan Prefecture are different. Therefore, HPV infection should pay more attention to the society and strengthen the routine detection of HPV. Infection, timely intervention.
