目的 探讨影响青春期发育的因素。方法 调查首次遗精 /月经初潮时间 ,测定血清瘦素、睾酮 /雌二醇 (E2 )、促卵泡成熟激素 (FSH)、黄体生成激素 (LH)、胰岛素水平 ,采用多元Logistic回归模型 ,对已遗精或有月经的 10 7例健康儿童及未遗精或无月经的 10 7例性别、年龄匹配儿童进行对照研究。结果 单因素分析显示 ,男性影响因素为身高、体重、胸围、臀围、血清瘦素、睾酮、FSH、LH、胰岛素 ;女性影响因素为身高、体重、胸围、臀围、血清瘦素、E2 、FSH、LH。多因素分析显示 ,体重 (OR =1.171,P <0 .0 1)、睾酮 (OR =1.0 10 ,P <0 .0 5 )、FSH (OR =1.5 0 5 ,P <0 .0 1)、血清瘦素 (OR =1.36 8,P =0 .0 0 1)为男性独立影响因素 ;体重 (OR =1.32 8,P <0 .0 0 1)、FSH (OR =1.173,P <0 .0 5 )、LH (OR =1.12 3,P <0 .0 5 )、血清瘦素 (OR =1.345 ,P <0 .0 0 1)为女性独立影响因素。结论 体重、FSH、血清瘦素均为男、女儿童青春期发育的独立影响因素 ,睾酮对男性青春期发育的影响十分突出 ,E2 对女性早期青春期发育的影响有待进一步研究。
Objective To explore the factors affecting the development of adolescence. Methods The levels of serum leptin, testosterone / estradiol (E2), follicle stimulating hormone (FSH), luteinizing hormone (LH) and insulin in the first spermatorrhea and menarche were investigated. Multivariate logistic regression Or 107 menstrual healthy children and no nocturnal fertility or without 107 cases of gender, age-matched children were controlled study. Results Univariate analysis showed that the factors influencing males were height, weight, chest circumference, hip circumference, serum leptin, testosterone, FSH, LH and insulin. The influencing factors were height, weight, chest circumference, hip circumference, serum leptin, E2, FSH, LH. Multivariate analysis showed that body weight (OR = 1.171, P <0.01), testosterone (OR = 1.010, P <0.05), FSH Serum leptin (OR = 1.36 8, P = 0.001) was an independent influencing factor in males. The body weight (OR = 1.328, P <0.01), FSH 5), LH (OR = 1.12 3, P <0.05), serum leptin (OR = 1.345, P <0.001) were independent influencing factors in women. Conclusion Body weight, FSH and serum leptin are both independent influencing factors of puberty development in male and female children. The influence of testosterone on male puberty development is very prominent. The effect of E2 on the development of female puberty remains to be further studied.
