目的观察血液灌流对急性重度有机磷农药中毒患者炎症因子的影响。方法将38l例符合急性重度有机磷农药中毒诊断标准的住院患者分为治疗组(26例)和对照组(12例)。检测并比较两组治疗前及治疗后6、24h血清中肿瘤坏死因子-(TNF-)、白细胞介素-1(IL-1)、-干扰素(IFN-)、白细胞介素-4(IL-4)水平。结果治疗前,两组TNF-、IL-1、IFN-、IL-4水平差异均无统计学意义(均>0.05);治疗6 h,治疗组TNF-、IL-1水平较对照组低(均<0.01),IFN-、IL-4水平亦较对照组低(均<0.05);治疗24 h,治疗组TNF-、IL-1水平较对照组低(均<0.05)。结论血液灌流可通过对炎症介质的清除作用,起到有效防治急性有机磷农药中毒的作用。
Objective To observe the effect of hemoperfusion on inflammatory cytokines in patients with acute severe organophosphorus pesticide poisoning. Methods 381 hospitalized patients who met the diagnostic criteria of acute severe organophosphorus pesticide poisoning were divided into treatment group (26 cases) and control group (12 cases). The level of TNF-α, IL-1, IFN-α, IL-4 and IL-6 in serum of the two groups before and 6 and 24 hours after treatment were detected and compared -4) level. Results Before treatment, there was no significant difference in the levels of TNF-, IL-1, IFN- and IL-4 between the two groups (all> 0.05). At 6 hours after treatment, the levels of TNF- and IL-1 in the treatment group were lower than those in the control group (All P <0.01). The levels of IFN- and IL-4 in the treatment group were also lower than those in the control group (all <0.05). After treatment for 24 h, the levels of TNF- and IL-1 in the treatment group were lower than those in the control group. Conclusion Hemoperfusion can effectively prevent acute organophosphorus pesticide poisoning by eliminating the inflammatory mediators.
