目的:探讨失效模式与效应分析(FMEA)在静脉输血流程改造及输血安全管理中的作用。方法:应用FMEA对静脉输血流程中的相关因数进行分析,计算出事件发生的RPN,并根据RPN的大小确定预防的重点,针对不安全因素不断调整流程、改进措施,使静脉输血流程安全系统不断完善并得到持续改进,并在1年后进行失效模式的RPN的比较。结果:运用FMEA对原有的静脉输血进行改进措施后,能识别输血流程中各潜在不安全因素及隐患,降低输血相关护理差错的发生率。结论:使用FMEA有效地降低了差错事故发生率,是一种行之有效的风险管理方法。
Objective: To explore the role of failure mode and effect analysis (FMEA) in the transfusion of venous transfusion and the safety of blood transfusion. Methods: FMEA was used to analyze the correlation coefficient of venous transfusion and the RPN of incident was calculated. According to the size of RPN, the focus of prevention was determined. The unsafe factors were continuously adjusted and improved so that the safety of intravenous transfusion flow was kept Perfected and continually improved, and compared the RPNs of failure modes one year later. Results: After using FMEA to improve the original venous blood transfusion, it can identify the potential unsafe factors and hidden dangers in the transfusion process and reduce the incidence of nursing-related errors in transfusion. Conclusion: The use of FMEA effectively reduces the incidence of error accidents and is an effective risk management method.
