目的 探讨实时超声弹性成像定量参数分析在鉴别诊断乳腺良恶性肿块中的价值.方法 分析168个乳腺肿块的超声弹性图像,计算周围腺体组织与病灶的应变比值(strain rate),分别统计良恶性病灶的比值均数,并通过ROC曲线分析取得最合适的临界值,由此分别计算敏感性、特异性、准确性.所有病例均经手术及病理证实.结果 良性肿块的应变比值为2.11±1.26,而恶性肿块的应变比值为8.51±5.93,两者之间的差异有统计学意义(P＜0.05),通过ROC曲线取得最合适的诊断临界点为3.12,以应变比值≥3.12为诊断恶性的标准,其诊断敏感性、特异性、准确性分别为85.71%、91.83、89.28%.结论 实时超声弹性成像定量参数分析为乳腺肿块的良恶性鉴别诊断提供了一种新的、更为客观、有价值的方法.
