为探讨T淋巴细胞亚群和Th1/Th2相关细胞因子在儿童特发性血小板减少性紫癜 (ITP)中变化及应用肾上腺皮质激素治疗对其的影响。用PMA、Ionomycin作刺激剂 ,刺激全血中淋巴细胞表达细胞因子 ,以Mon ensin阻断细胞因子分泌至胞外 ,应用两种免疫荧光抗体同时标记淋巴细胞的膜表面特异分子和表达被阻滞在细胞内的细胞因子 ,用流式细胞仪进行检测和分析。结果 :ITP患儿外周血CD4 +细胞降低 ,CD8 +细胞升高 ,CD4 +/CD8 +降低尤为显著。治疗前Th1细胞占(20.66±2.82) %,Th2细胞占 (4.55±2.33) % ,Th1/Th2比值为4.78±0.66,明显高于正常儿童的1.43±0.19(P<0.005),治疗后与正常儿童无差异。提示ITP患儿T淋巴细胞亚群表达及比例失调 ,IFN_γ产生增多 ,TL_4产生减少 ,Th1/Th2功能异常 ;ITP是一种Th1占优势的疾病 ;肾上腺皮质激素治疗ITP能有效缓解病情
To investigate the changes of T lymphocyte subsets and Th1 / Th2-related cytokines in children with idiopathic thrombocytopenic purpura (ITP) and the effects of corticosteroid therapy on them. PMA and Ionomycin were used as stimulants to stimulate the expression of cytokines in lymphocytes in whole blood. Monensin was used to block the secretion of cytokines to the extracellular domain. Results: The levels of CD4 + cells, CD8 + cells and CD4 + / CD8 + in peripheral blood of ITP children decreased significantly. Th1 cells accounted for (20.66 ± 2.82)%, Th2 cells accounted for (4.55 ± 2.33)%, Th1 / Th2 ratio was 4.78 ± 0.66 before treatment, which was significantly higher than that of normal children (1.43 ± 0.19, P <0.005) No difference for children. Suggesting that ITP children with T lymphocyte subsets expression and dysregulation, increased IFNγ production, TL_4 production decreased, Th1 / Th2 dysfunction; ITP is a predominant Th1 disease; corticotropin treatment of ITP can effectively alleviate the disease
