目的 :探讨开窍醒神汤联合奥拉西坦对老年重型颅脑损伤患者脑保护作用以及炎症反应的影响。方法:选取2014年11月—2015年11月在我院接受治疗的老年重型颅脑损伤患者84例,采用随机数表法将患者分为观察组和对照组,每组各42例,对照组给予奥拉西坦治疗,观察组在此基础上给予开窍醒神汤治疗,观察两组患者治疗前后NHISS评分、MMSE评分、BI指数以及炎症因子变化情况,同时比较两组患者治疗后不良反应发生率以及临床效果。结果 :治疗后,两组患者的NHISS评分均有所降低,观察组(15.45±3.56)分低于对照组(18.12±5.02)分,两组比较差异显著(P<0.05);治疗后,两组患者的MMSE评分均有所升高,观察组(21.02±4.89)分高于对照组(18.02±5.02)分,两组比较差异显著(P<0.05);治疗后,两组患者的BI指标均有所升高,观察组高于对照组,两组比较差异显著(P<0.05);治疗后,两组患者的IL-6、TNF-α以及hs-CRP均有所降低,观察组(46.12±11.02)ng/L、(32.08±8.56)pg/m L、(13.65±3.45)mg/L低于对照组(52.45±9.68)ng/L、(39.12±8.89)pg/m L(19.14±5.02)、mg/L,两组比较差异显著(P<0.05);治疗后,观察组不良反应总发生率11.96%低于对照组30.95%,两组比较差异显著(P<0.05);治疗后,观察组总有效率95.23%高于对照组76.19%,两组比较差异显著(P<0.05)。结论 :给予老年重型颅脑损伤患者开窍醒神汤联合奥拉西坦治疗,能够有效改善患者的脑神经功能,降低炎症因子的水平表达,稳定患者的病情,有助于改善患者的预后,效果显著。
