目的评价彩色多普勒超声对医源性股动脉假性动脉瘤诊治的应用价值。方法应用线阵高频探头对15例临床疑诊有股动脉假性动脉瘤患者进行彩色多普勒超声检查,观察其形态、大小、内部回声及血流频谱特征。对其中明确诊断患者12例在超声引导下进行手指局部压迫30~60分钟后用弹力绷带加压,平卧8小时。结果15例患者当中12例在穿刺部位出现囊性包块,呈搏动性,囊内有涡流彩色血流信号,囊颈部血流频谱呈“双期双向”,确诊为假性动脉瘤,对这12患者在超声引导下进行手指压迫,11例一次压迫成功,1例重复压迫成功,无并发症发生。结论彩色多普勒超声能够诊断假性动脉瘤,对明确诊断的患者在超声引导下手指压迫修复治疗是一种安全、可靠、简便的方法。
Objective To evaluate the value of color Doppler ultrasound in diagnosis and treatment of iatrogenic femoral artery pseudoaneurysm. Methods Fifteen cases of suspected pseudoaneurysm with femoral artery aneurysm were examined by color Doppler ultrasonography. The morphology, size, internal echo and the frequency spectrum of blood flow were observed. Twelve of the patients with definite diagnosis were under pressure-guided local compression for 30 to 60 minutes and then pressed with elastic bandages for 8 hours. Results Among the 15 patients, 12 cases had cystic mass at the puncture site, which showed pulsatile color signals of vortex flow in the capsule. The frequency spectrum of blood flow in the capsule neck was “two-phase bidirectional” and was diagnosed as pseudoaneurysm. The 12 patients underwent finger-pressing under the guidance of ultrasound. One patient underwent a successful compression and one patient underwent repeated compression without complications. Conclusions Color Doppler ultrasound can diagnose pseudoaneurysm, and it is a safe, reliable and simple method for the diagnosis of patients under the guidance of ultrasound guided finger compression repair.
