目的胃肠道间质瘤是胃肠道最常见的非上皮源性肿瘤,可发生在从食管到直肠的消化道的任何部位。探讨腹腔镜联合消化内镜治疗胃肠道间质瘤的临床效果。方法术前准备同开腹手术,清洁胃肠道,采用气管插管静脉复合全麻,术中胃镜或肠镜预先镜下切除,或定位后腹腔镜切除肿瘤,回顾性分析2008～2010年间双镜联合治疗胃肠间质瘤的手术方法及临床效果。结果 10例患者中2例胃间质瘤患者直接胃镜下切除剥离7,例胃镜定位下腹腔镜切除缝合。1例结肠平滑肌瘤患者镜下治疗后肠道穿孔,腹腔镜补加手术治疗。术后患者恢复好,痊愈出院。结论应用腹腔镜及胃肠镜联合治疗胃肠道间质瘤具有定位准确迅速、缩短手术时间、手术创伤小、术后疼痛轻、恢复快等优点,具有良好的应用价值。
Purpose Gastrointestinal stromal tumors are the most common non-epithelial tumors of the gastrointestinal tract that can occur anywhere in the digestive tract from the esophagus to the rectum. To investigate the clinical effect of laparoscopy combined with digestive endoscopy in the treatment of gastrointestinal stromal tumors. Methods Preoperation was performed with laparotomy, laparoscopic resection of the gastrointestinal tract, general anesthesia with endotracheal intubation, preoperative endoscopic gastroscopy or colonoscopy, or laparoscopic resection of the tumor. Combined surgical treatment of gastrointestinal stromal tumors and its clinical effect. Results Of the 10 patients, 2 cases of gastric stromal tumors underwent direct endoscopic resection and dissection 7, and underwent endoscopic laparoscopic resection and suture. One patient with colon leiomyoma underwent endoscopic perforation and laparoscopic surgery. Postoperative patients recovered well, discharged. Conclusion The combination of laparoscopy and gastrointestinal endoscopy in the treatment of gastrointestinal stromal tumors has the advantages of rapid and accurate positioning, shortened operation time, less trauma, less postoperative pain and rapid recovery, and has good application value.
