目的:探讨螺旋CT对食管癌临床应用价值,并与传统CT相比较。材料与方法:螺旋CT组食管癌12例均行多平面重建,并与35例行传统CT扫描者进行轴位图像比较;7例螺旋CT扫描后在工作站经软件处理行食管仿真内窥镜成像。结果:(1)12例食管癌螺旋CT扫描与传统CT扫描轴位图像所见几乎完全一致,可以观察病变形态,侵及范围,了解有无转移情况等,与手术病理相符。而螺旋CT多平面重建可以沿食管走行进行曲面多平面重建,以最佳角度,不同方位来观察食管肿瘤位置、周围受浸范围。(2)7例食管癌CT仿真内窥镜可从头端或足端入路观察食管壁及肿瘤在腔内的改变等,与纤维食管胃镜所见一致。结论:螺旋CT与传统CT相比,除了可以轴位观察食管病变情况,还可以利用多平面重建及CT仿真内窥镜来观察食管病变特征,为诊断提供更多,更全面的信息。CT信真内窥镜也是纤维食管胃镜的重要补充方法。
Objective: To investigate the clinical application value of spiral CT in esophageal cancer and compare it with traditional CT. Materials and Methods: Multi-planar reconstruction was performed in 12 patients with esophageal carcinoma in spiral CT, and axial images were compared with 35 patients who underwent conventional CT scan. Seven cases of spiral CT scan were performed at the workstation by software. . Results: (1) The spiral CT scans of 12 cases of esophageal cancer were almost identical to those seen on the axial images of traditional CT scans. It was possible to observe the lesion morphology, extent of invasion, and to understand the presence or absence of metastases, which was consistent with the surgical pathology. The multi-planar reconstruction of spiral CT can be performed along the esophagus to perform multi-planar reconstruction of the surface, to observe the location of the esophageal tumor and the extent of the surrounding immersion at the best angle and orientation. (2) 7 cases of esophageal cancer CT virtual endoscope can observe the change of esophageal wall and tumor in the cavity from the head-end or foot-end approach, which is consistent with the findings of fiberoptic esophagoscopy. Conclusion: Compared with traditional CT, spiral CT can be used to observe esophageal lesions in axial position. Multiplanar reconstruction and CT virtual endoscopy can also be used to observe the characteristics of esophageal lesions, providing more and more comprehensive information for diagnosis. CT telescopes are also an important supplement to fiberoptic esophagoscopy.
