目的:探讨高压氧(HBO)对咽喉肿瘤术后创口延迟愈合的治疗作用。方法:口咽癌、喉咽癌、喉癌手术治疗后伤口延迟愈合患者83例,其中48例行HBO综合治疗(HBO综合治疗组),压力取2×105Pa,110min/次,每天1次,5~10次为1个疗程。35例常规换药综合治疗进行对照(常规换药组)。结果:HBO治疗3~4次后,创面即有新鲜肉芽生成,其后纤维肉芽和毛细血管不断生成充填创面,直至创口愈合,其中皮瓣坏死愈合平均时间为27.50d,咽瘘为8.50d,创腔积液或感染为5.93d;常规换药组皮瓣坏死为45.00d,咽瘘为14.09d,创腔积液或感染为8.62d。2组平均愈合时间比较均差异有统计学意义(均P<0.01)。结论:HBO对咽喉肿瘤术后伤口延迟愈合患者有明显的治疗作用,可促进伤口愈合。
Objective: To investigate the therapeutic effect of hyperbaric oxygen (HBO) on delayed wound healing in patients with throat tumor. Methods: 83 cases of delayed wound healing were treated by oropharyngeal carcinoma, laryngopharyngeal carcinoma and laryngeal carcinoma. Among them, 48 cases were treated with HBO combined therapy (HBO combined treatment group), the pressure was 2 × 105Pa, 110min / time, 5 to 10 times for a course of treatment. 35 cases of conventional dressing general treatment control (conventional dressing group). Results: After 3 or 4 times of HBO treatment, fresh granulation was formed on the wound surface, and then granulation and capillaries of fibrous granulation continued to fill the wound until wound healing. The average time for necrosis of skin flap was 27.50 days and pharyngeal fistula was 8.50 days, Fluid effusion or infection was 5.93d; conventional dressing group flap necrosis was 45.00d, pharyngeal fistula was 14.09d, invasive fluid or infection was 8.62d. The average healing time in both groups showed statistically significant difference (all P <0.01). Conclusion: HBO has a significant therapeutic effect on delayed wound healing in patients with pharyngolaryngeal neoplasms and can promote wound healing.
