目的:探讨β-catenin、P53和增殖细胞核抗原在胃腺上皮癌变过程中的表达。方法:采用免疫组织化学EnVision法对观察组(58例胃腺上皮低级别上皮内瘤变、64例胃腺上皮高级别上皮内瘤变、32例胃腺癌变标本)和对照组(20例正常胃粘膜标本)β-catenin、P53和增殖细胞核抗原表达进行检测,观察β-catenin、P53和增殖细胞核抗原在正常胃粘膜→胃腺上皮低级别上皮内瘤变→胃腺上皮高级别上皮内瘤变→胃腺癌变的演变过程表达。结果:20例正常胃粘膜组织的对照组细胞β-catenin表达可见细胞膜呈棕黄色,细胞浆和细胞核无着色,未见P53蛋白的阳性表达,细胞PCNA呈现弱阳性表达。胃腺瘤从胃腺上皮低级别上皮内瘤变到胃腺癌变在β-catenin细胞浆和细胞核共同表达阳性率及在细胞浆表达阳性率均出现逐渐递增(P<0.01),胃腺瘤从胃腺上皮低级别上皮内瘤变到胃腺癌变P53蛋白、细胞PCNA表达明显递增(P<0.01),经Spearman等级相关性分析,β-catenin、P53和PCNA在胃腺上皮癌变过程中表达呈显著正相关(P<0.01)。结论:β-catenin、P53和PCNA在胃腺瘤癌变的过程中发挥了至关重要的作用,可作为预测病变程度及评估患者预后的有效指标。
Objective: To investigate the expression of β-catenin, P53 and proliferating cell nuclear antigen in gastric carcinogenesis. Methods: Immunohistochemistry EnVision method was used to observe the changes in the observation group (58 cases of gastric epithelial low grade intraepithelial neoplasia, 64 cases of gastric epithelial high grade intraepithelial neoplasia, 32 cases of gastric adenocarcinoma) and control group (20 cases of normal gastric mucosa ) β-catenin, P53 and proliferating cell nuclear antigen expression were detected in the observation of β-catenin, P53 and proliferating cell nuclear antigen in the normal gastric mucosa → gastric epithelial low grade intraepithelial neoplasia → gastric epithelial high-grade intraepithelial neoplasia → gastric adenocarcinoma Evolutionary process expression. Results: The expression of β-catenin in 20 normal gastric mucosa cells showed brownish yellow cell membrane, no staining of cytoplasm and nucleus, no positive expression of P53 protein and weak positive expression of PCNA. Gastric adenomas from gastric epithelial low-grade intraepithelial neoplasia to gastric adenocarcinoma in β-catenin cytoplasm and nucleus co-expression positive rate and positive expression in the cytoplasm showed a gradual increase (P <0.01), gastric adenoma from the gastric epithelium low level The intraepithelial neoplasia changed the expression of P53 protein and the expression of PCNA in gastric adenocarcinoma (P <0.01). There was a significant positive correlation between the expression of β-catenin, P53 and PCNA in gastric carcinogenesis by Spearman rank correlation (P <0.01) ). Conclusion: β-catenin, P53 and PCNA play a crucial role in the carcinogenesis of gastric adenocarcinoma and can be used as an effective index to predict the degree of disease and evaluate the prognosis of patients.
