目的:探讨三维超声在胎儿的全前脑和面部畸形诊断中的临床应用价值。方法:将2008年1月—2014年10月在我院通过孕期保健检查并且在初诊时可疑胎儿结构的2 000名孕妇作为本研究的对象,采用二维和三维超声检查,对纳入检查的病例结果、引产结果进行了对比分析。结果:在胎儿颜面的诊断中二维超声和三维超声对颜面部的确诊准确率为87.0%、92.4%,对比差异无统计学意义(P>0.05),另外对颜面畸形细节的判断准确率为82.6%、88.0%,差异亦无统计学意义(P>0.05);在全前脑畸形的判断上看,二维超声检测对经引产及出生证实的诊断符合率为96.97%,而通过三维超声行胎儿全前脑畸形诊断符合率为99.25%,但差异无统计学意义。结论:三维超声对比二维超声看,对颜面部畸形和全前脑畸形的准确率更高,诊断的敏感度和特异性好,具有较好的临床应用价值,值得在临床中广泛推广。
Objective: To investigate the clinical value of three-dimensional ultrasonography in the diagnosis of fetal anterior total brain and facial deformity. Methods: A total of 2 000 pregnant women, who passed the pregnancy health examination in our hospital from January 2008 to October 2014 and were suspected to have fetal structure at the time of first visit, were enrolled in this study. Two-dimensional and three-dimensional ultrasonography Results, induced labor results were compared. Results: The diagnostic accuracy of two-dimensional ultrasound and three-dimensional ultrasound on facial was 87.0% and 92.4% respectively in the diagnosis of fetal facial surface. There was no significant difference between the two groups (P> 0.05). The accuracy of facial deformity was 82.6% and 88.0%, respectively. There was no significant difference between the two groups (P> 0.05). Judging from the anterior cerebral malformations, the coincidence rate of two-dimensional ultrasonography in the diagnosis of induced labor and birth was 96.97% Line fetal diagnosis of anterior cerebral malformations was 99.25%, but the difference was not statistically significant. Conclusion: Compared with two-dimensional ultrasound, the accuracy of three-dimensional ultrasound is superior to facial deformity and total anterior cerebral deformity. The diagnostic sensitivity and specificity are good. It has good clinical value and is worth widely popularized in clinic.
