目的通过对深圳市龙岗区0~6岁健康儿童开展肠道病毒71型(EV71)血清流行病学调查,了解辖区儿童对EV71的易感性,从而为手足口病防控提供科学依据。方法于2011年上半年及2012年上半年入学体检期间,收集托幼机构入托体检健康儿童血液样本464份,采用ELISA法进行EV71 IgG和IgM抗体检测。结果 464份健康托幼儿童血样中,血清EV71 IgG阳性率和IgM阳性率分别为46.12%(214/464)和1.51%(7/464)。1岁以下儿童EV71 IgG阳性率(24.78%,28/113)低于1~6岁儿童(52.99%,186/351)(P<0.01),但EV71 IgM阳性率在2组间的差异无统计学意义(Fisher’s exact test,P>0.05)。男童EV71 IgG、IgM阳性率分别为47.84%(122/255)和2.35%(6/255),女童EV71 IgG、IgM阳性率分别为44.02%(92/209)和0.48%(1/209),男童与女童的EV71 IgG阳性率及IgM阳性率差异均无统计学意义(均P>0.05)。不同街道健康托幼儿童EV71 IgG阳性率与辖区手足口病暴发疫情密度(r=0.90,P<0.01)、手足口病聚集性疫情密度(r=0.84,P<0.05)相关,但与辖区手足口病报告发病率(r=0.11,P>0.05)、重症发病率(r=-0.67,P>0.05)相关不显著。结论深圳市龙岗区仍有过半健康儿童对EV71易感,3岁以下儿童尤其是1岁以下儿童为EV71感染高风险人群。
Objective To investigate the epidemiological investigation of enterovirus 71 (EV71) in healthy children aged 0 ~ 6 years in Longgang District, Shenzhen to understand the susceptibility of children in the area to EV71 so as to provide a scientific basis for prevention and control of hand-foot-mouth disease. Methods During the first half of 2011 and the first half of 2012, 464 blood samples were collected from nurseries and kindergartens, and EV71 IgG and IgM antibodies were detected by ELISA. Results The positive rates of serum EV71 IgG and IgM in 464 healthy children were 46.12% (214/464) and 1.51% (7/464), respectively. The positive rate of EV71 IgG in children younger than 1 year (24.78%, 28/113) was lower than that in children aged 1-6 years (52.99%, 186/351) (P <0.01), but the positive rate of EV71 IgM was not statistically different between the two groups Significance (Fisher’s exact test, P> 0.05). The prevalence of EV71 IgG in healthy children and kindergartens in different streets was related to the density of outbreaks of HFMD in the area (r = 0.90, P <0.01) and the aggregated epidemic density of HFMD (r = 0.84, P < The incidence of mouth disease reported (r = 0.11, P> 0.05), the incidence of severe disease (r = -0.67, P> 0.05) was not significant. Conclusion There are still more than half of healthy children in Longgang District, Shenzhen susceptible to EV71, children under 3 years of age, especially children under the age of 1, EV71 infection in high-risk groups.
