目的了解徐州地区妇女生殖道人乳头瘤病毒(HPV)感染情况。方法采用凯普医用核酸分子杂交技术及其配套试剂对2009年2月-2012年5月于我院体检、就诊的39 997例女性行生殖道人乳头瘤病毒分型检测,分析HPV感染及亚型分布情况。结果 (1)39 997例女性中HPV阳性8 369例,阳性率20.92%;(2)其中单一感染6 305例,占阳性病例的75.34%,多重感染2 064例,占阳性病例的24.66%;单一感染与多重感染比较差异有统计学意义(χ2=5 723.03,P<0.0001);(3)感染的高峰年龄为20岁以下组,感染率30.21%。随年龄增长,HPV感染率逐渐下降,51~60岁组出现HPV感染的第二个高峰,感染率为21.82%;(4)HPV高危亚型感染7 079例,低危亚型感染2 116例,两者比较差异有统计学意义,(χ2=2931.16,P<0.0001);(5)随年龄的增加HPV高危亚型的感染率逐渐升高,低危亚型的感染率逐渐降低,各年龄组间单一高危型感染率差异具统计学意义,(χ2=55.45,P<0.0001)。(6)21种HPV亚型均被检出,感染率排前六位的亚型为16、58、52、53、CP8304、11。其中HPV16感染率与其他各型相比差异有统计学意义,P<0.0001。HPV58感染率与其他各型相比差异有统计学意义(P<0.01)。结论徐州地区女性HPV感染率较高,以单一、高危型感染为主,HPV16、58、52、53是主要的感染亚型。
Objective To understand the prevalence of human papillomavirus (HPV) infection in reproductive tract of women in Xuzhou area. Methods 39 997 female reproductive tract human papillomavirus (HPV) genotypes were detected in our hospital from February 2009 to May 2012 in our hospital from February 2009 to May 2012. The HPV infection and subtype Distribution. Results (1) There were 8 369 HPV positive cases in 39 997 women, with a positive rate of 20.92%. (2) Among them, 6 305 were single infections, accounting for 75.34% of the positive cases and 2064 cases of multiple infections, accounting for 24.66% of the positive cases. The difference between single infection and multiple infection was statistically significant (χ2 = 5 723.03, P <0.0001). (3) The peak infection age was less than 20 years old and the infection rate was 30.21%. With the increase of age, the infection rate of HPV decreased gradually, and the second peak of HPV infection occurred in 51-60 age group, the infection rate was 21.82%. (4) 7079 cases of high risk HPV subtypes, 2,116 cases of subtype of low risk infection (Χ2 = 2931.16, P <0.0001). (5) With the increase of age, the infection rate of HPV high-risk subtype gradually increased, the infection rate of low-risk subtype gradually decreased, and all ages The single high-risk infection rate between groups was statistically significant (χ2 = 55.45, P <0.0001). (6) Twenty-one HPV subtypes were detected, and the top six infection subtypes were 16,58,52,53 and CP8304,11. Among them, HPV16 infection rate had statistical significance compared with other types, P <0.0001. HPV58 infection rate compared with other types of difference was statistically significant (P <0.01). Conclusion The prevalence of HPV infection in Xuzhou women is high, mainly in single and high-risk type. HPV16,58,52 and 53 are the main infection subtypes.
