为了解下呼吸道细菌感染抗生素经验治疗与细菌分离结果一致性,以Kappa值评价,Kappa值≥0.4为一致性较好,反之为一致性差.85例下呼吸道细菌感染的病原菌包括革兰阴性杆菌(GNB)50例(58.8%),革兰阳性球菌(GPC)35例(41.2%).初选抗生素以抗GNB为主者55例(64.7%),以抗GPC为主者30例(35.3%),大致与病原菌构成相符.但其Kappa值均<0.4,显示选用药物的抗菌谱与分离的病原菌一致性较差.提示临床应注意提高分析致病菌种类的能力.
In order to understand the experience of antibiotic treatment of lower respiratory tract bacterial infection and consistency of bacterial isolation results, the Kappa value evaluation, Kappa value ≥ 0.4 for consistency is good, whereas the consistency is poor .85 cases of lower respiratory tract bacterial pathogens include Gram-negative bacilli ( GNB), 35 cases (41.2%) of Gram-positive cocci (GPC), 55 cases (64.7%) of primary antibiotics with anti-GNB and 30 cases (35.3% ), Roughly consistent with the constitution of pathogenic bacteria, but its Kappa values were less than 0.4, indicating that the antibacterial spectrum of the selected drugs is not consistent with the isolated pathogens, suggesting that the clinical should pay attention to improving the ability of analyzing pathogenic bacteria.
