目的:探讨CD123在不同类型成熟B淋巴细胞肿瘤诊断和鉴别诊断中的意义。方法:选取297例成熟B淋巴细胞肿瘤患者作为研究对象,疾病类型包括慢性淋巴细胞白血病/小淋巴细胞淋巴瘤(CLL/SLL),套细胞淋巴瘤(MCL),滤泡性淋巴瘤(FL),脾边缘区淋巴瘤(SMZL),毛细胞白血病(HCL)和变异型HCL(HCLv)。采集患者的骨髓标本,通过流式细胞术分析其免疫表型,以及各类型疾病中异常淋巴细胞群CD123表达的抗原阳性比例及强度。结果:不同类型成熟B淋巴细胞肿瘤患者骨髓异常淋巴细胞群中,其CD123表达率及表达强度存在较大差异。其中156例CLL/SLL中CD123阳性比例约占3.21%,表达强度较低;45例MCL中CD123阳性比例约占8.89%,表达强度较高;62例FL中CD123阳性比例约占4.84%,表达强度较低;20例SMZL CD123阳性比例约占15.00%,表达强度较高;10例HCL CD123阳性比例占100%,且其表达强度最高;但4例HCLv未见CD123阳性表达。结论:CD123在成熟B淋巴细胞肿瘤常见类型中广泛表达,其表达率和强度尤其可作为HCL与其他亚类进行鉴别的重要指标之一。
Objective: To investigate the significance of CD123 in the diagnosis and differential diagnosis of different types of mature B lymphocytic tumors. METHODS: Totally 297 patients with mature B lymphocytic tumors were selected as study subjects. The disease types included chronic lymphocytic leukemia / small lymphocytic lymphoma (CLL / SLL), mantle cell lymphoma (MCL), follicular lymphoma (FL) , Splenic marginal zone lymphoma (SMZL), hairy cell leukemia (HCL), and variant HCL (HCLv). Bone marrow samples from patients were collected and their immunophenotypes were analyzed by flow cytometry and the antigen-positive ratio and intensity of CD123 expression in the abnormal lymphocyte population in each type of disease. Results: There was a significant difference in the expression of CD123 and the expression of CD123 among different types of patients with myeloid leukemia. Of 156 cases, the positive rate of CD123 in CLL / SLL was 3.21% and the expression intensity was lower. The positive rate of CD123 in 45 cases was 8.89%, and the positive rate was higher in 62 cases. The positive rate of CD123 in 62 cases was 4.84% 20 cases of SMZL CD123 positive rate of about 15.00%, higher expression intensity; 10 cases of HCL CD123 positive ratio accounted for 100%, and the highest expression intensity; but 4 cases of HCLv no CD123 positive expression. CONCLUSION: CD123 is widely expressed in the common types of mature B lymphocytic tumors, and its expression rate and intensity can be used as one of the important indexes for distinguishing HCL from other subtypes.
