目的评价复合保温对腹腔镜异位妊娠手术患者应激反应的影响。方法选择行腹腔镜异位妊娠患者168例,随机分成A,B两组。其中A组为复合保温组(实验组),B组为常规保温组(对照组),每组84例。分别记录入室前后鼻咽温度T,两组入室后肾上腺素和去甲肾上腺素含量,及送化验科检测凝血功能(分别检测PT,APTT,FBG)。结果①两组患者年龄、性别、体重、气腹压力、气腹时间、输液量、冲洗量、室温等组间比较差异无统计学意义(P>0.05);②术后A组麻醉复苏时间短。③A组患者肾上腺素气腹后和术毕升高的幅度比B组小,两组比较,差异有统计学意义(P>0.05)。A组患者气腹后的去甲肾上腺素明显低于B组,术毕比较差异无统计学意义(P>0.05);④两组患者入室的PT,APTT,FBG比较差异无统计学意义(P>0.05),术毕均延长,A组延长的幅度小,术毕与入室相比差异无统计学意义(P>0.05)。B组延长的幅度大,术毕明显高于入室水平。组间比较,术毕各项指标B组明显高于A组差异有统计学意义(P>0.05)。结论围术期患者应用复合保温措施有利维持异位妊娠腹腔镜手术中体温的稳定,减轻腹腔镜手术中的应激反应。
Objective To evaluate the effect of composite insulation on stress response in patients undergoing laparoscopic ectopic pregnancy. Methods 168 patients with laparoscopic ectopic pregnancy were randomly divided into A and B groups. Among them, group A was a composite thermal insulation group (experimental group) and group B was a conventional thermal insulation group (control group), with 84 cases in each group. The nasopharyngeal temperature T before and after admission, the adrenaline and norepinephrine levels in the two groups were recorded, and the coagulation function was detected in the laboratory (PT, APTT and FBG respectively). Results ① There was no significant difference in age, gender, body weight, pneumoperitoneum pressure, pneumoperitoneum time, infusion volume, irrigation volume and room temperature between the two groups (P> 0.05). ② The recovery time of A group after operation was short . ③A group of patients with pneumoperitoneum after pneumoperitoneum and the rising rate of surgery is less than the B group, the two groups, the difference was statistically significant (P> 0.05). The pneumoperitoneum norepinephrine in group A was significantly lower than that in group B, and there was no significant difference after operation (P> 0.05). ④ The PT, APTT and FBG in the two groups had no significant difference (P > 0.05). All the patients had prolonged operation. The extension of A group was small, there was no significant difference between the two groups (P> 0.05). B group extended a large extent, surgery was significantly higher than the entry level. The differences between groups were statistically significant (P> 0.05). Conclusion Perioperative patients with composite insulation measures to maintain ectopic pregnancy laparoscopic surgery in the stability of body temperature and reduce the stress response during laparoscopic surgery.
