目的对医院门急诊药房调配中常见的差错原因进行分析,并提出相应的防范措施,减少调配差错的发生。方法选取医院药品调配差错登记本中记录的164份,对其差错原因进行分析,并依据差错原因提出相应的防范措施。结果 164份药品调配差错登记中总结调配差错原因主要药品品种、剂量剂型、药品数量、包装相似、用法用量、处方、注射单书写、收费、发出药品不合格、错发等差错,其中药品品种、剂量剂型差错所占比例较高。结论针对差错原因进行分析,可通过完善药品调配流程、加强药师专业水平、加强药品的管理三方面进行防范,从而减少门急诊药房调配差错的发生。
Objective To analyze the common causes of errors in outpatient and emergency department pharmacy deployment and put forward corresponding preventive measures to reduce the occurrence of deployment errors. Methods A total of 164 records were recorded in the register of errors in the allocation of medicines in hospitals.
