目的观察妊娠期妇女血脂水平对新生儿体重、身长和头围的影响,探讨孕妇高血脂与巨大儿出生率的关系。方法于2014年3月~6月,对新疆奎屯市某医院妇产科分娩单胎的产妇101例及101例未孕健康妇女,分别检测血清总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL)、低密度脂蛋白胆固醇(LDL)、载脂蛋白AI(Apo AI)及载脂蛋白B(Apo B)水平,并追踪新生儿出生体重、身长,头围和胸围,分析孕晚期血脂变化及其对新生儿出生体重的影响。结果孕妇孕晚期血脂水平比非妊娠妇女高(P<0.001),孕晚期血清总胆固醇(TC)水平随新生儿出生体重增加而上升,两者呈正相关(r=0.478,P<0.001),新生儿出生体重与母亲HDL值呈负相关(r=-0.318,P=0.001),新生儿出生体重与母亲Apo AI值呈负相关(r=-0.298,P=0.002),高血脂组孕妇的新生儿出生体重高于正常血脂组(P<0.001)。孕妇孕晚期的TC水平越高,新生儿出生体重越重(P<0.001)。结论孕期血脂水平高于非妊娠健康妇女,产妇孕晚期高血脂水平对新生儿出生体重会造成影响。
Objective To observe the effect of serum lipids in pregnant women on body weight, body length and head circumference of neonates, and explore the relationship between hyperlipidemia and the birth rate of macrosomia in pregnant women. Methods From March to June 2014, 101 pregnant women and 101 healthy pregnant women who gave birth to singletons in obstetrics and gynecology department of a hospital in Kuitun, Xinjiang were enrolled in this study. Serum total cholesterol (TC), triglyceride (TG) HDL, LDL, Apo AI and Apo B levels were measured and the newborn’s birth weight, length, head and chest circumference , Analysis of the changes of blood lipid in the third trimester and its effect on newborn birth weight. Results The serum levels of TC in pregnant women at the third trimester were higher than those in non pregnant women at the third trimester (P <0.001). The levels of total cholesterol (TC) in the second trimester of pregnant women increased with the birth weight of the newborns. There was a positive correlation between them (r = 0.478, There was a negative correlation between birth weight and mothers HDL (r = -0.318, P = 0.001). Neonatal birth weight was negatively correlated with mothers Apo AI (r = -0.298, P = 0.002) The birth weight was higher in children than in normal lipids group (P <0.001). The higher the level of TC in pregnant women in the third trimester, the heavier the newborn’s birth weight (P <0.001). Conclusion The level of blood lipid in pregnancy is higher than that in non-pregnant healthy women. The level of serum lipids in late pregnancy may affect the birth weight of newborns.
