一起“飞车”事故的原因分析

我段东风_4型0522号机车于1984年5月4日牵引416次旅客列车,从怀化站出发,至象鼻子站预告信号机前回手柄瞬间,柴油机转速急剧上升,声音骤变。乘务员意识到发生了“飞车”,立即断开操纵台开关,但柴油机仍以很高的转速惰转了几十秒钟后才停机。经外观检查,除前通风机尼龙轴严重破损外,未见异状。再行起动,未能奏效,以致造成机破。机车回段后,经外观及解体检查,主要发现下列问题:前后通风机叶片全部脱落;排气门杆全部弯曲;排气门导管全部破损;前、后增压器转子及喷嘴环破损。这次严重“飞车”事故所造成的直接经济损失就达三万五千余元之多。 I paragraph Dongfeng _4 0522 locomotive on May 4, 1984 traction 416 passenger trains, starting from Huaihua Station, to the nose station notice signal before the handle back to the moment, the rapid rise of diesel engine speed, the sound suddenly changed. The direct economic losses caused by this serious “speeding car accident” amounted to more than 35,000 yuan.
