我段东风_4型0522号机车于1984年5月4日牵引416次旅客列车,从怀化站出发,至象鼻子站预告信号机前回手柄瞬间,柴油机转速急剧上升,声音骤变。乘务员意识到发生了“飞车”,立即断开操纵台开关,但柴油机仍以很高的转速惰转了几十秒钟后才停机。经外观检查,除前通风机尼龙轴严重破损外,未见异状。再行起动,未能奏效,以致造成机破。机车回段后,经外观及解体检查,主要发现下列问题:前后通风机叶片全部脱落;排气门杆全部弯曲;排气门导管全部破损;前、后增压器转子及喷嘴环破损。这次严重“飞车”事故所造成的直接经济损失就达三万五千余元之多。
