INTRODUCTION Conventional lactate-based peritoneal dialysis solu-tions (L-PDS) are unphysiologic fluids, which have beenemployed in continuous ambulatory peritoneal dialysis(CAPD) for over two decads. Novel experimentalpyruate-based peritoneal dialysis solutions (P-PDS) wererecently found to be superior in biocompatibility. It ispossible that P-PDS would be a potent alternative in thenear future. The present review discusses effects of bothlactate and pyruvate, and high glucose on human leuko-cytes, mainly on intracellular pH ([pH]_i), glucosemetabolism, and apoptosis, to elucidate advantages ofpyruvate in peritoneal dialysis solutions (PDS) and itsprobable mechanisms of the cell protection.
