目的:分析和总结营养支持在老龄胃癌患者围手术期应用的意义。方法:63例老龄胃癌患者在术前1周内给予肠内营养或肠外营养,入院时、手术前后分别检测血前白蛋白、转铁蛋白与24h尿氮含量。结果:本组患者入院时前白蛋白、转铁蛋白较对照组明显下降(P<0.01);经营养支持后手术前前白蛋白、转铁蛋白较入院时明显上升但仍低于对照组,术后第7天时前白蛋白、转铁蛋白较术前下降不明显。本组病例与同期入院55岁以下胃癌患者术后并发症发生率及平均住院时间的差异无显著性意义。结论:对老龄胃癌患者进行围手术期营养支持可有效改善患者的营养状况,减少术后并发症发生。
Objective: To analyze and summarize the significance of nutritional support in the perioperative application of elderly patients with gastric cancer. Methods: 63 cases of elderly patients with gastric cancer were given enteral nutrition or parenteral nutrition within one week before operation. Pre-operative and postoperative serum albumin, transferrin and 24-hour urinary nitrogen were measured. Results: Prealbumin and transferrin were significantly lower than those of the control group before admission (P <0.01). The levels of albumin and transferrin before operation were significantly higher than those of the control group after operation support, Preoperative albumin after 7 days, transferrin less than preoperative decreased significantly. No significant difference was found in the incidence of postoperative complications and the average length of hospital stay between patients in this group and patients with gastric cancer under 55 in the same period. Conclusion: Perioperative nutritional support for elderly patients with gastric cancer can effectively improve the nutritional status of patients and reduce the incidence of postoperative complications.
