[目的]探讨特发性血小板减少性紫癜(ITP)患者骨髓巨核细胞和血小板4项参数的变化及临床意义。[方法]分析经我院确治的99例ITP患者骨髓巨核细胞和血小板数(Plt)、血小板平均体积(MPV)、血小板平均分布宽度(PDW)、大血小板比率(P-LCR)的关系及比较治疗前后血小板4项参数变化。[结果]①骨髓巨核细胞数和Plt没有相关性,但颗粒型、产板型巨核细胞和Plt有一定相关性(P﹤0.01)。②Plt与MPV呈负相关(P﹤0.01);MPV与PDW、P-LCR,PDW与P-LCR呈正相关(P﹤0.05)。③ITP患者治疗前Plt明显低于对照组,而MPV、PDW、P-LCR明显高于对照组,差异有统计学意义(P﹤0.001);治疗后Plt上升MPV、PDW、P-LCR下降与治疗前比较差异有统计学意义(P﹤0.001)。[结论]观察血小板参数对骨髓巨核细胞产血小板功能及病情判断、疗效观察有重要意义。
[Objective] To investigate the changes of 4 parameters of bone marrow megakaryocytes and platelets in patients with idiopathic thrombocytopenic purpura (ITP) and its clinical significance. [Methods] The relationship between bone marrow megakaryocytes and the number of platelets (Plt), mean platelet volume (MPV), average platelet width (PDW) and platelet ratio (P-LCR) Compare the four parameters of platelet before and after treatment. [Results] ① There was no correlation between the number of megakaryocytes and Plt in the bone marrow, but there was a correlation between the number of megakaryocytes and the type of platelet and platelet (P <0.01). There was a negative correlation between PIt and MPV (P <0.01). There was a positive correlation between MPV and PDW, P-LCR, PDW and P-LCR (P <0.05). (3) PIt in patients with PI before treatment was significantly lower than that in control group, but MPV, PDW and P-LCR were significantly higher than those in control group (P <0.001); After treatment, Plt increased MPV, PDW, P-LCR decreased and treatment Before the difference was statistically significant (P <0.001). [Conclusion] It is of great significance to observe the function and condition of platelets derived from platelet megakaryocytes and observe the effect of platelet parameters.
