目的探索社区2型糖尿病管理的模式,提高管理效率,提升治疗效果。方法随机抽取本社区内的948名2型糖尿病患者,并将其随机分成实验组与对照组。对照组采用一般门诊随访方式进行;实验组采用分组管理的模式随访。对两组研究对象管理后自我管理能力及糖代谢指标的改善情况进行比较。结果两组研究对象在伴随的慢性非传染性疾病的分布上无差别。但是,实验组研究对象的糖代谢指标、自我管理能力均较对照组有明显改善。结论分组管理是一种适合在社区开展的糖尿病管理模式,能够调动患者参与管理的积极性,有效舒缓患者的情绪,能更好地促进患者做到合理用药、合理膳食、适宜的运动锻炼,从而有效改善2型糖尿病的预后、延缓并发症发生、提高生活质量,并提高患者的依从性。
Objective To explore the model of community type 2 diabetes management, improve the management efficiency and improve the treatment effect. Methods A total of 948 type 2 diabetic patients in our community were randomly selected and divided into experimental group and control group randomly. The control group was followed by general outpatient visits; the experimental group was followed up by group management. After the two groups of subjects management self-management ability and improvement of glucose metabolism indicators were compared. Results There was no difference between the two groups in the distribution of the accompanying chronic noncommunicable diseases. However, the subjects of the experimental group had significantly improved indicators of glucose metabolism and self-management as compared with the control group. Conclusion Group management is a kind of diabetes management mode suitable for the community, which can mobilize the enthusiasm of patients to participate in management, effectively relieve the emotion of the patients, and can better promote the rational use of drugs, reasonable diet and appropriate physical exercises so as to be effective Improve the prognosis of type 2 diabetes, delay the occurrence of complications, improve the quality of life, and improve patient compliance.
