目的:观察糖尿病肾病(DN)模型大鼠肾小管上皮细胞标志蛋白CK-18的表达,探讨通络益肾汤对DN模型大鼠肾脏保护作用的机理。方法:采用单侧肾脏切除+STZ诱导DN大鼠模型,大鼠随机分为正常对照组、模型组、通络益肾汤组、西药治疗组。观察各组大鼠治疗后证候积分、空腹血糖(FBG)、肾功能(BUN、Cre)、24 h尿蛋白定量(24 h Upr)及肾小管上皮细胞标志蛋白CK-18的表达。结果:与模型组比较,通络益肾汤组大鼠的证候积分、FBG、BUN、Cre、24 h Upr均明显下降(P<0.05或P<0.01),肾小管上皮细胞CK-18表达明显升高(P<0.05)。在改善证候积分、恢复肾小管上皮细胞CK-18表达方面,通络益肾汤组明显优于西药治疗组(P<0.05)。结论:通络益肾汤对DN大鼠模型具有一定的治疗作用,其机理可能与提高DN模型大鼠肾小管上皮细胞CK-18表达,抗肾纤维化有关。
OBJECTIVE: To observe the expression of tubular epithelial cell marker protein CK-18 in diabetic nephropathy (DN) model rats and to explore the mechanism of Tonglu Yishen Decoction on renal protection in DN model rats. Methods: Unilateral nephrectomy + STZ-induced DN rats were randomly divided into normal control group, model group, Tonglu Yishen decoction group and western medicine group. The scores of syndromes, fasting blood glucose (FBG), renal function (BUN, Cre), 24 h urinary protein (24 h Upr) and renal tubular epithelial cell marker CK-18 were observed after treatment in each group. Results: Compared with the model group, the scores of syndrome points, FBG, BUN, Cre and 24 h Upr in Tongluanyishen decoction group were significantly decreased (P <0.05 or P <0.01), and CK-18 expression in renal tubular epithelial cells Was significantly higher (P <0.05). Tonglu Yishen Decoction group was significantly better than western medicine group (P <0.05) in improving syndrome score and restoring renal tubular epithelial cells CK-18 expression. Conclusion: Tonglu Yishen Decoction has a certain therapeutic effect on DN rat model. The mechanism may be related to increasing the expression of CK-18 in renal tubular epithelial cells and anti-renal fibrosis in DN model rats.
