目的:探讨妊娠合并梅毒早期发现、早期诊断并早期治疗对改善妊娠结局及围产儿预后的临床意义,降低先天性梅毒的发生率。方法:82例血清学检查确诊为梅毒的孕妇,6例流产,其余76例根据妊娠期梅毒发现时间及治疗情况分为全程治疗组(45例)、非全程治疗组(20例)及未治疗组(11例),比较3组孕妇的妊娠结局及先天性梅毒儿的发生情况,观察治疗时期、疗程与新生儿先天性梅毒发生率的关系。结果:全程治疗组足月分娩率为95.6%(43/45),高于非全程治疗组(80%,8/10)和未治疗组(54.5%,6/11),3组相比差异有统计学意义(P<0.01);未治疗组发生死胎2例,非全程治疗组发生死胎1例;未治疗组新生儿先天性梅毒发生率为81.8%(9/11),高于非治疗组(30%,6/20)和全程治疗组(11.1%,5/45),3组相比差异有统计学意义(P<0.01)。结论:加强流动孕产妇管理、早期诊断及早期规范治疗对改善合并梅毒感染的孕产妇的妊娠结局、降低围产儿死亡率及新生儿先天性梅毒发生率具有重要意义。
Objective: To investigate the clinical significance of early detection, early diagnosis and early treatment of pregnancy complicated with syphilis in pregnancy to improve pregnancy outcomes and prognosis of perinatal women, and to reduce the incidence of congenital syphilis. Methods: Eighty - two pregnant women diagnosed as syphilis by serology were included in the study, and 6 were aborted. The remaining 76 cases were divided into treatment group (45 cases), non - treatment group (20 cases) and untreated group (N = 11). The pregnancy outcomes of congenital syphilis and the incidence of congenital syphilis were compared between the three groups to observe the relationship between the treatment period and the incidence of congenital syphilis. Results: The full-term delivery rate was 95.6% (43/45) in the whole treatment group, which was higher than that in the non-treatment group (80%, 8/10) and the untreated group (54.5%, 6/11) (P <0.01). There were 2 stillbirths in untreated group and 1 stillbirth in non-complete treatment group. The incidence of congenital syphilis in untreated group was 81.8% (9/11), which was higher than that of non-treatment group (30%, 6/20) and the whole treatment group (11.1%, 5/45), the difference between the three groups was statistically significant (P <0.01). CONCLUSION: It is of great significance to improve the pregnancy outcomes of pregnant women with syphilis infection and to reduce perinatal mortality and the incidence of congenital syphilis in infants by enhancing the management, early diagnosis and early standardization of mobile pregnant women.
