[目的]探讨西维来司钠对重症急性胰腺炎大鼠炎性因子及谷丙转氨酶(ALT)、谷草转氨酶(AST)表达的影响.[方法]选择SD大鼠30只,随机分为对照组、模型组、干预组.对照组经腹正中切口并显露胰腺、十二指肠,不做夹闭操作,模型组及干预组采用经十二指肠乳头逆行胰胆管注射5%牛黄胆酸钠的方法建立急性胰腺炎的模型,对照组和模型组经颈静脉置管微量泵注射生理盐水,干预组给予西维来司钠治疗.干预后6h和12 h时,对胰腺组织进行病理评分,检测并比较各组血清中炎性因子白细胞介素-2 (IL-2)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)及ALT、AST水平.[结果]建模后6h、12 h时,模型组大鼠的胰腺组织病理学评分及血清淀粉酶(AMY)、ALT、AST、IL-2、IL-6、IL-8含量均显著高于对照组(t=10.129~21.664,P<0.01),干预组大鼠的胰腺组织病理学评分以及血清AMY、ALT、AST、IL-2、IL-6、IL-8含量均显著低于模型组(t=6.055~12.551,P< 0.05,P<0.01).[结论]西维来司钠能减轻急性胰腺炎大鼠的胰腺病理损伤、抑制炎症反应、保护肝功能.“,”[Objective]To study the effect of sivelestat on the expression of inflammatory factors,alanine aminotransferase (ALT) and aspartate aminotransferase (AST) in rats with severe acute pancreatitis.[Methods]A total of 30 SD rats were divided into control group,model group and intervention group.Rats in the control group underwent a ventral midline incision to exposure pancreas and duodenum without clamping.Severe acute pancreatitis (SAP) was established in the model group and the intervention group by retrograde injection of 5 % sodium taurocholate from the duodenal papilla into the biliopancreatic duct.The control group and model group were injected with saline solution by jugular vein catheter,and the intervention group received the sivelestat treatment.The levels of serum inflammatory factors (IL-2,IL-6 and IL-8),ALT and AST were measured at 6h and 12h after SAP initiation.At the same time points,the pathological score of pancreatic tissue was evaluated.[Results]At 6h and 12h after SAP initiating,the pathological score of pancreatic tissue and serum levels of amylase (AMY),ALT,AST,IL-2,IL6 and IL-8 in the model group rats were significantly higher than those in the control group(t =10.129 ~ 21.664,P <0.01).However,the pathological score of pancreatic tissue and serum levels of AMY,ALT,AST,IL-2,IL-6 and IL8 in the intervention group were significantly lower than those in the model group (t =6.055 ~ 12.551,P <0.01).[Conclusion]Sivelestat can alleviate pancreatic damage of severe acute pancreatitis in rat,inhibit inflammation,and protect liver function.
