目前,全国80%的卫生人才集中在大中城市。在经济欠发达的县级医院和乡镇卫生院基本上没有本科生。甘肃省的东乡县有27万人口,卫生技术人员只有219人,其中本科2人,大专12人,中专205人,全县每千人口拥有卫生技术人员仅为0.81人。河南省全省乡镇卫生院的在职人员中本科学历仅占0.89%,而且绝大多数都是后续本科学历。这种趋势还在加剧。在卫生行业流传这样一句话:“乡卫生院为县医院培养人才,县医院为市医院培养人才,市医院为省会医院和发达城市医院培养人才”。这充分说明了人才逆向流动的严重程度和无奈。在村一级卫生室,几乎全部为中专以下或无学历的乡村医生和卫生员。在国家已对农民新型农村合作医疗进行补助的情况下,直接为逾8亿农民服务的乡村医生所提供的仍然是以谋生为前提的卫生服务,绝大多数未享受任何补贴。

There are basically no undergraduates in economically underdeveloped county-level hospitals and township hospitals. While the state has subsidized farmer new-type rural cooperative medical services, rural doctors who directly serve more than 800 million peasants are still provided health services on the basis of earning their livelihood. Most of them do not receive any subsidies.
