目的探讨研究美国国立综合癌症网络(National Comprehensive Cancer Network,NCCN)心理痛苦干预对中晚期肺癌患者行为功能状态和生活质量的影响。方法选取我院2012年3月~2015年11月收治确诊的154例中晚期肺癌病患,对纳入研究的全体患者采取随机数字表法将其平均分成观察组与对照组,每组各77例。其中对照组实施常规性护理,观察组则在此护理基础上施以NCCN心理痛苦干预,观察比较两组患者的护理前后的行为功能状态量表(KPS)评分,心理痛苦温度计(DT)评分与心理痛苦相关因素调查表(PL)评分、生命质量测定量表(QLQ-C30)评分。结果两组患者实施护理干预前上述量表评分组间比较差异无统计学意义(P>0.05);实施护理干预后上每组患者的上述量表评分均较干预前明显好转,且差异有统计学意义(P<0.05);观察组干预后KPS评分、PL评分及DT的痛苦检出率均明显较对照组为优(P<0.05)。两组患者护理干预后QLQ-C30评分中,只有经济困难维度评分组间比较差异无统计学意义(P>0.05),剩余各项评分观察组较对照组均显著为优(P<0.05)。结论 NCCN心理痛苦干预可更有效的缓解中晚期肺癌的痛苦,改善其生活质量与行为功能状态。
Objective To investigate the effect of psychological pain intervention in American National Comprehensive Cancer Network (NCCN) on behavioral functional status and quality of life in advanced lung cancer patients. Methods A total of 154 patients with advanced lung cancer diagnosed in our hospital from March 2012 to November 2015 were randomly divided into observation group and control group, with 77 cases in each group . The control group received routine nursing, and the observation group received NCCN psychological pain intervention on the basis of this nursing. The KPS score, psychological pain thermometer (DT) score and Psychological pain related factors survey (PL) score, quality of life scale (QLQ-C30) score. Results There was no significant difference between the two groups before nursing intervention (P> 0.05). The score of each scale in each group after nursing intervention was significantly better than that before intervention, and the difference was statistically significant (P <0.05). The KPS score, PL score and the painful detection rate of DT in the observation group after intervention were significantly higher than those in the control group (P <0.05). Among the QLQ-C30 scores after nursing intervention in both groups, there was no significant difference between the scores of the economic difficulty scale (P> 0.05), and the remaining scores in the observation group were significantly better than the control group (P <0.05). Conclusion NCCN psychological pain intervention can more effectively alleviate the pain of advanced lung cancer and improve their quality of life and behavioral functional status.
