目的探讨气道内悬挂125Ⅰ粒子治疗中心型肺癌引起的气道狭窄和肺不张的方法和疗效。方法采用环甲膜穿刺、置管、气道内悬挂125Ⅰ放射性微粒子方法,对中心型肺癌进行持续近距离放疗。治疗观察时间3个月。结果21例患者中,气道狭窄CR 5例、PR 10例、AC 4例、PD 2例,CR+PR 15(72%)。并发颈部瘘口炎性肉芽组织增生、喉部不适、导管刺激性咳嗽各3例,血WBC下降低于正常2例,导丝在颈部折断1例。结论经环甲膜穿刺,气道内悬挂125Ⅰ粒子对于治疗肺癌引起气道狭窄和肺不张的方法简便、安全和有效。
Objective To investigate the method and effect of airway suspension 125Ⅰ particles in the treatment of central lung cancer caused by airway stenosis and atelectasis. Methods The method of cricothyroid membrane puncture, catheterization and suspension of 125 I radioactive particles in the airway was used to perform continuous brachytherapy on central lung cancer. Treatment observation time of 3 months. Results Among the 21 patients, 5 were CR, 10 were PR, 4 were AC, 2 were PD, and CR + PR 15 was 72%. Concomitant neck fistula inflammatory granulation tissue hyperplasia, throat discomfort, catheter irritating cough in 3 cases, blood WBC decreased below normal in 2 cases, the guide wire in the neck fracture in 1 case. CONCLUSION: It is a simple, safe and effective method to treat airway stenosis and atelectasis caused by lung cancer with 125 Ⅰ particles suspended in the airway.
