目的观察水平牵张成骨技术治疗火器伤牙槽突缺损的效果。方法因鞭炮炸伤导致严重牙槽突缺损1例,义齿修复受到影响,行水平牵张成骨治疗。手术矩形截除18 mm×12 mm死骨,遗留骨缺损采用水平骨牵开进行修复。设计宽约15 mm的骨转移盘,术后第8天开始加力。结果患者顺利完成治疗,牙槽突恢复长度约16 mm,X线显示成骨良好。结论水平牵张成骨技术可以弥补垂直牵张成骨方法的不足,治疗火器伤牙槽突缺损效果满意。
Objective To observe the effect of horizontal distraction osteogenesis in the treatment of alveolar process defect of firearm. One case of severe alveolar process defect caused by firecrackers’ injury was repaired, and the denture repair was affected. Horizontal distraction osteogenesis was performed. Rectangular surgical resection of 18 mm × 12 mm sequestrum, left bone defects using horizontal distraction for repair. Design a bone transfer plate with a width of 15 mm and apply the force on the 8th postoperative day. Results The patients completed the treatment successfully. The length of the alveolar process recovered about 16 mm and the X-ray showed good osteogenesis. Conclusion Horizontal distraction osteogenesis can make up for the lack of vertical distraction osteogenesis, and the effect of alveolar process in treating firearm injuries is satisfactory.
