目的探讨膝关节滑膜软骨瘤病国内外诊治经验并文献复习。方法笔者于2015-04诊治1例膝关节巨大滑膜软骨瘤并重度骨关节炎,采用人工膝关节表面置换+滑膜切除+游离体摘除术治疗。结果病理检查确诊为滑膜软骨瘤病,经1年随访膝关节功能良好,无感染、复发等不良事件发生。结论本例术中所摘除软骨瘤体积巨大,且合并重度骨关节炎,较为罕见,诊治上颇为特殊,需引起重视。X线片、CT、MRI及高频超声在诊断上均具有特征性,金标准仍系病理检查;膝关节滑膜软骨瘤并骨关节炎的治疗需综合病理分级、骨关节炎严重程度等因素,以期获得良好疗效。
Objective To investigate the diagnosis and treatment of knee synovial chondromatosis at home and abroad and review the literature. Methods The author in 2015-04 diagnosis and treatment of a case of giant synovial chondroma knee and severe osteoarthritis, the use of artificial knee joint surface replacement + synovial resection + free extirpation treatment. Results Pathological examination was diagnosed as synovial chondromatosis. After 1 year of follow-up, knee joint function was good, no infection and recurrence occurred. Conclusion In this case, the size of chondroma excised is huge, and it is rare to have severe osteoarthritis. The diagnosis and treatment are rather special and need to be taken seriously. X-ray, CT, MRI and high-frequency ultrasound in the diagnosis of both characteristic, the gold standard is still pathological examination; knee synovial chondroma and osteoarthritis treatment need comprehensive pathological grade, the severity of osteoarthritis and other factors , In order to obtain good curative effect.
