[目的]研究Vibrant动态增强MRI在早期乳腺癌诊断中的应用价值。[方法]回顾性分析经手术病理证实的27例早期乳腺癌患者的Vibrant动态增强MRI图像,分别从病灶形态学和血流动力学两方面进行分析评估其临床价值。[结果]27例患者共检出27个病灶,所有图像脂肪抑制均匀,病灶显示清晰。Vibrant动态增强MRI形态学多表现为结节状、腺管状或丛状强化,边缘呈蟹足状、星芒状或毛刺征,与周围结构分界不清;时间—信号强度曲线Ⅰ型2例(7.4%),Ⅱ型7例(25.9%),Ⅲ型18例(66.7%)。以形态不规则、星芒状或毛刺征和Ⅱ、Ⅲ型曲线作为乳腺癌的诊断标准,其敏感度分别为70.4%、74.1%和92.6%。[结论]Vibrant动态增强MRI能较好地显示病灶的形态学及血流动力学信息,对早期乳腺癌的检出及正确诊断具有重要价值。
[Objective] To investigate the value of dynamic VVI in the diagnosis of early breast cancer. [Methods] The Vibrant dynamic contrast-enhanced MRI images of 27 patients with early-stage breast cancer confirmed by surgery and pathology were retrospectively analyzed. The clinical value was analyzed from the aspects of lesion morphology and hemodynamics. [Results] A total of 27 lesions were detected in 27 patients. All the images showed uniform fat suppression and clear lesions. The morphology of Vibrant dynamic contrast-enhanced MRI mostly showed nodular, glandular or plexiform enhancement with marginal crab-like, star-like or burr sign and unclear boundary with the surrounding structures. Time-signal intensity curve type I (2 cases) 7.4%), type Ⅱ 7 cases (25.9%), type Ⅲ 18 cases (66.7%). The results showed that the sensitivity was 70.4%, 74.1% and 92.6%, respectively, for the diagnostic criteria of breast cancer with irregular shape, stellate or burr sign and type Ⅱ and Ⅲ curves. [Conclusion] Vibrant dynamic contrast-enhanced MRI can better display the morphology and hemodynamic information of lesions and is of great value in the detection and correct diagnosis of early breast cancer.
