目的探讨血清炎症因子白细胞介素18(IL-18)、白细胞介素1β(IL-1β)与多囊卵巢综合征(PCOS)发病的关系。方法2006年10月至12月对北京大学第三医院住院的PCOS伴有胰岛素抵抗患者30例、PCOS不伴胰岛素抵抗患者30例及对照组30例,检测血清空腹血糖(FBG)、空腹胰岛素(FINS)、卵泡刺激素(FSH)、黄体生成素(LH)和睾酮(T);酶联免疫吸附试验(ELISA)方法检测血清IL-18、IL-1β。结果PCOS胰岛素抵抗组IL-18为(243.16±64.23)ng/L,IL-1β为(158.49±29.60)ng/L;非胰岛素抵抗组血清IL-18为(174.27±66.78)ng/L,IL-1β为(85.58±29.20)ng/L,均明显高于对照组[(122.44±40.15)ng/L,(45.84±26.64)ng/L],差异有显著性意义(P<0.05);PCOS胰岛素抵抗组血清IL-18、IL-1β明显高于PCOS非胰岛素抵抗组(P<0.05)。PCOS肥胖组IL-18为(274.46±63.58)ng/L、IL-1β为(149.35±35.97)ng/L,明显高于PCOS非肥胖组[(172.47±63.95)ng/L,(96.49±41.55)ng/L](P<0.05)。体重指数(BMI)正常的PCOS组血清IL-18和IL-1β明显高于BMI正常的对照组(P<0.05)。PCOS组血清IL-18、IL-1β与BMI、HomaIR及T呈显著正相关(P<0.05)。结论炎症因子IL-18、IL-1β在PCOS患者血清中明显升高,并且在胰岛素抵抗和肥胖的PCOS患者中升高更为明显。慢性炎症可能参与PCOS的发病,并且与胰岛素抵抗及肥胖有关。
Objective To investigate the relationship between serum interleukin 18 (IL-18), interleukin-1β (IL-1β) and polycystic ovary syndrome (PCOS). Methods From October 2006 to December 2006, 30 PCOS patients with PCOS, 30 PCOS patients without insulin resistance and 30 patients with PCOS were enrolled in the Third Hospital of Peking University. The fasting blood glucose (FBG), fasting insulin FINS, follicle stimulating hormone (FSH), luteinizing hormone (LH) and testosterone (T), and serum IL-18 and IL-1β by enzyme-linked immunosorbent assay (ELISA) Results The serum level of IL-18 in PCOS group was (243.16 ± 64.23) ng / L, and that of IL-1β was (158.49 ± 29.60) ng / L in PCOS group and 174.27 ± 66.78 ng / L in non-insulin resistance group 1β was (85.58 ± 29.20) ng / L, both of which were significantly higher than that of the control group [(122.44 ± 40.15) ng / L, (45.84 ± 26.64) ng / L] Serum levels of IL-18 and IL-1βin insulin resistance group were significantly higher than those in PCOS non-insulin resistance group (P <0.05). The level of IL-18 in PCOS obesity group was (274.46 ± 63.58) ng / L, and that of IL-1β was (149.35 ± 35.97) ng / L, significantly higher than that in non-obese PCOS group (172.47 ± 63.95 ng / L, 96.49 ± 41.55 ) ng / L] (P <0.05). Serum levels of IL-18 and IL-1β in PCOS group with normal body mass index (BMI) were significantly higher than those in normal BMI group (P <0.05). Serum levels of IL-18 and IL-1β in PCOS group were positively correlated with BMI, HomaIR and T (P <0.05). Conclusion The inflammatory cytokines IL-18 and IL-1β are significantly elevated in serum of patients with PCOS, and are more obviously elevated in patients with insulin resistance and obesity. Chronic inflammation may be involved in the pathogenesis of PCOS, and insulin resistance and obesity.
