作者采用心内膜导管起搏时的体表心电图QRS波与自发室性心动过速(VT)时的体表心电图QRS波形进行比较,作为VT起源点的定位方法,并以电消蚀VT成功为金标准,对8例VT患者进行定位研究并评价。结果显示心内膜起搏时心电图QRS波形有9个导联以上与自发VT时QRS波符合时,起搏点在VT起源点附近,直流电消蚀成功,射频电则不行。当两者心电图QRS 12个导联符合时,起搏点即VT起源点,射频消蚀成功,表明我们采用的VT起源点定位方法是准确可行的。
The authors compared endocardial QRS waveforms during autologous ventricular tachycardia (VT) with endocardial catheter pacing and QRS waveforms of body surface electrocardiography (ECG) as a method of locating the origin of VT and succeeded in electrically erasing VT As the gold standard, eight patients with VT were located and evaluated. The results showed that there were more than 9 lead QRS waveforms in echocardiographic pacing and QRS wave spontaneous VT when the pace point in the vicinity of VT origin, DC erosion success, the radio frequency is not. When the two ECG QRS lead line 12 coincide, the pace point is the VT origin, RF ablation success, indicating that we use the VT origin point positioning method is accurate and feasible.
