目的:研究膀胱尿路上皮癌(BUC)组织中Paxillin蛋白的表达情况,探讨Paxillin蛋白与BUC临床病理参数及淋巴结转移的相关性,以及对患者生存预后的影响.方法:收集我院209例BUC行根治性膀胱切除术后病理标本及临床病理等资料,同时随机选取40例癌旁正常膀胱组织病理标本及40例阳性淋巴结作为对照.检测Paxillin蛋白表达情况,分析Paxillin表达同BUC患者淋巴结转移等临床指标之间的关系,利用单因素及多因素分析BUC患者发生淋巴结转移的影响因素,以及根治术后的预后影响因素.结果:膀胱尿路上皮癌组织中Paxillin表达水平明显高于正常膀胱组织,阳性表达率分别为82.8%和27.5% (P<0.01).Paxillin表达水平与肿瘤的直径、数量、淋巴结转移、肿瘤分级、临床分期显著相关(P<0.05),Logistic回归分析提示Paxillin表达水平为BUC发生淋巴结转移的独立预测因素(P<0.05).Paxillin蛋白高表达组患者的术后无复发生存时间及总生存时间明显低于低表达组(P<0.05).结论:Paxillin蛋白在BUC组织中相对于正常膀胱组织高表达,是预测淋巴结转移的影响因素.Paxillin蛋白高表达是影响BUC患者预后的重要指标.“,”Objective:To investigate the expression of Paxillin protein in bladder urothelial carcinoma (BUC) tissues,explore the correlation between Paxillin protein and BUC clinicopathological parameter,lymph node metastasis and the effect of expression of Paxillin protein on the prognosis of patients.Method:We collected the data of pathological specimen and clinical pathology,which come from 209 BUC patients who underwent radical cystectomy in our hospital.We selected 40 pathological specimens of para-carcinoma normal bladder tissue and 40 positive lymph nodes as control randomly.We examined the expression of Paxillin protein,analysed the relationship of clinical indexes between the expression of Paxillin protein and BUC patients with lymph node metastasis,used univariate and multivariate analysis to analyse the influencing factors of lymph node metastasis in BUC patients,as well as the prognostic factors after radical cystectomy.Result:The expression level of Paxillin protein in BUC was significantly higher than that in normal bladder tissues,the positive expression rates were 82.8% and 27.5 % respectively (P<0.01).The expression level of Paxillin had a dramatical connection with the diameter of tumor,quantity,lymph node metastasis and clinical stages.Logistic regression analysis showed that only the expression level of Paxillin protein was an independent risk factor in BUC patients with lymph node metastasis (P<0.05).The postoperative relapse-free survival time and overall survival of high expression group of Paxillin protein were significantly lower than those of low expression group (P<0.05).Conclusion:Compared with normal bladder tissues Paxillin protein highly expressed in BUC tissues,which is an influencing factor to predict lymph node metastasis.High expression of Paxillin protein is an important indicator of the prognosis in BUC patients.
