Objective To understand the level of community-based knowledge and policy about unpaid blood donation and to explore the influencing factors of willingness to pay blood donation. Methods In January 2008, 3 natural villages were randomly selected from 3 villages in northern Anhui, Anhui and southern Anhui in Anhui Province. The questionnaires were collected by self-made questionnaires. 724 interviewees were interviewed and 620 valid questionnaires were obtained. The effective response rate 85.6%. Results The relative knowledge score of unpaid blood donation in rural residents was 19.26 ± 3.987, and the relevant policy score was 5.85 ± 2.154. There were significant differences in scores of knowledge related to different age groups. There were significant differences in scores of relevant policies among different age and education levels. Logistic analysis showed that age, education level and the level of awareness of relevant policies were the factors affecting the willingness of rural residents to pay blood donation. Conclusion The level of knowledge about unpaid blood donation in rural areas is relatively high and the level of awareness about relevant policies is relatively low. The willingness to pay for blood donation is significantly higher among 25-34-year-olds than that of 18-24-year-olds. The higher the level of awareness, the higher the willingness to pay for blood donations.
